内镜下诊断及治疗早期胃癌的研究进展

Research progress on diagnosis and treatment of early gastric cancer under endoscopy

随着内镜技术的不断发展,早期胃癌的诊断技术也在不断成熟,除了白光内镜,目前染色内镜、放大内镜、超声内镜、窄带成像技术、共聚焦激光显微内镜等也得到了广泛应用,不同检查方式的联合应用大大提高了早期胃癌的检出率。而早期胃癌的治疗方式也在不断发展,目前内镜下黏膜剥离术的临床应用最为广泛,一些新的内镜下治疗方式也在不断探索和完善中,衍生出了内镜下全层胃壁切除术、腹腔镜联合内镜手术、非暴露的内窥镜翻壁手术等新术式,为早期胃癌的治疗提供了新的思路。

With the continuous development of endoscopy technology,the diagnosis technology of early gastric cancer is also maturing.In addition to white light endoscopy,the current dyeing endoscopy,magnifying endoscopy,ultrasound endoscopy,narrowband imaging technology,confocal laser microendoscopy,etc.It has been widely used,and the combined application of different inspection methods has greatly improved the detection rate of early gastric cancer.The treatment of early gastric cancer is also constantly developing.At present,the clinical application of endoscopic mucosal dissection is the most widely used.Some new endoscopic treatment methods are also constantly being explored and improved,resulting in endoscopic full-thickness gastric wall resection.New surgical procedures,such as laparoscopic combined endoscopic surgery,provide new ideas for the treatment of early gastric cancer.
