针刺治疗原发性失眠选穴规律

Acupoint Selection Pattern of Primary Insomnia Acupuncture Treatment

目的:基于复杂网络技术,探讨针刺治疗原发性失眠的核心腧穴及其之间的配伍关系。方法:检索从建库至2019年9月31日收录在中国期刊全文数据库、万方、维普、中国生物医学文献网中针刺治疗原发性失眠的相关文献,经Note Express 3.2.0软件进行文件管理和筛选,将符合条件的针刺处方录入Microsoft Excel,建立数据库,运用Gephi 0.8.2构建针刺治疗原发性失眠腧穴处方的复杂网络模型,分析其拓扑结构,发现核心腧穴及其之间的配伍规律。结果:符合要求的腧穴处方82条;构建的针刺治疗原发性失眠腧穴处方的复杂网络具有小世界效应和无标度特性,腧穴节点度呈幂律分布;根据核心腧穴的节点重要性,穴位按度值由高到低分别为百会、四神聪、太冲、内关、足三里、三阴交、神门、合谷、安眠、印堂、神庭、太溪,腧穴配伍以百会配神门、百会配安眠、神门配三阴交最为常见;核心腧穴的属性分析,督脉腧穴个数及使用频次最高,分布以头顶、颈部、前额为多,66.67%的核心腧穴为特定穴,以五输穴最多。结论:针刺治疗原发性失眠的处方中,穴位配伍以局部取穴或局部配远端(远近配穴)取穴最为常用;特定穴是其核心腧穴的核心。

Objective:To explore core acupoints and their concerted application patterns in the treatment of primary insomnia based on complex network technology.Methods:Retrieve relevant literature on acupuncture treatment of primary insomnia in CNKI,Wanfang,VIP,and CBM from the establishment of the database to September 31,2019.Related documents were managed and screened by Note Express 3.2.0 software,and the qualified acupuncture prescriptions were imported into Microsoft Excel to build a database.Gephi 0.8.2 was used to construct a complex network model of acupuncture prescriptions in primary insomnia treatment,and its topology Structure was analyzed to discover its core acupoints and its concerted application patterns.Results:82 acupoint prescriptions meeting the requirements.And the established complex network has the characteristics of small-world effect and scalefree properties,the degrees of acupoints obeying power-law distribution.According to their importance,the core acupoints are Baihui(GV20),Sishencong(EX-HN1),Taichong(LR3),Neiguan(PC 6),Zusanli(ST36),Sanyinjiao(SP6),Shenmen(HT7),Hegu(LI4),Anmian,Yintang(EX-HN3),Shenting(DU24),Taixi(KI3)in descending order of degrees.The most common concerted application patterns are The compatibility of acupoints is Baihui with Shenmen,Baihui with Anmian,and Shenmen with Sanyinjiao;And according to the attribute analysis,the governor vessel has the highest usage in number and frequency,which mostly distributed on the top of head、neck and forehead.And 66.67%of them are special acupoints,of which the five-Shu points are the majority.Conclusion:In the prescription of acupuncture for primary insomnia,distal-proximal points combination is the most commonly used while special acupoints are the essence of the core acupoints.
