家庭医生签约服务下医护团队组合对社区高血压患者管理效果分析

Analysis on the effect of medical team combination on community hypertension patient management under contracted family doctor service

目的:探讨社区高血压患者管理中应用家庭医生签约服务下医护团队组合的价值。方法:选取2018年10月-2019年10月社区高血压患者100例,随机分为两组,各50例。对照组实施常规管理;观察组实施家庭医生签约服务下医护团队组合管理。比较两组干预效果。结果:观察组干预后收缩压、舒张压水平均低于对照组,差异有统计学意义(P<0.05);观察组高血压知识知晓率及自我管理能力评分均高于对照组,差异有统计学意义(P<0.05)。结论:针对高血压患者应用家庭医生签约服务下医护团队组合模式,可有效控制患者血压水平,增强高血压知识知晓率,提高自我管理能力。

Objective:To explore the value of medical team combination under the contract service of family doctor in the management of hypertension patients in the community.Methods:From October 2018 to October 2019,100 patients with hypertension in the community were selected and randomly divided into two groups with 50 patients in each group.The control group received conventional management.The observation group was managed by the combination of medical and medical teams under the contracted service of family doctors.The effect of intervention was compared between the two groups.Results:The levels of systolic and diastolic blood pressure in the observation group were lower than the control group after intervention,with the differences were statistically significant(P<0.05).The knowledge rate and self-management score of hypertension in the observation group were higher than the control group,with the difference was statistically significant(P<0.05).Conclusion:For patients with hypertension,the combination mode of medical team under the contract service of family doctor can effectively control patients'blood pressure level,enhance the awareness rate of hypertension knowledge and improve self-management ability.
