家庭医生签约服务在社区高血压慢病管理中的应用价值及可行性分析

Application value and feasibility analysis of family doctor's contracted service in community hypertension and chronic disease management

目的:分析家庭医生签约服务在社区高血压慢病管理中的应用价值及可行性。方法:选取2019年1-12月社区高血压患者100例,随机分为两组,各50例。对照组采取常规管理方法;观察组选择家庭医生签约服务方法。比较两组应用效果。结果:观察组收缩压、舒张压水平均低于对照组,差异有统计学意义(P<0.05);观察组酗酒率、高盐高脂饮食率及心脑血管病发生率均低于对照组,适度运动率、遵医行为率、控制情绪率及服药依从性均高于对照组,差异有统计学意义(P<0.05)。结论:针对社区高血压患者管理过程中,采用家庭医生签约服务规范化管理的方法,对于控制血压水平和改善生活方式有很好的推进作用。

Objective:To analyze the application value and feasibility of the contracted service method of family doctors in the management of chronic diseases for patients with hypertension in the community.Methods:From January to December 2019,100 cases of patients with hypertension who treated in the community were enrolled,they were randomly divided into two groups with 50 cases in each group.The control group adopted routine management method.The observation group chose family doctor's contract service method.The applied effects of two groups was compared.Results:The systolic and diastolic blood pressure levels in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The rates of alcohol abuse,high salt and high fat diet and the incidence of cardiovascular and cerebrovascular diseases in the observation group were lower than those in the control group,moderate exercise rate,compliance rate,control mood rate and drug compliance were higher than those in control group,the differences were statistically significant(P<0.05).Conclusion:The standardized management of family doctor's contract service in the management of community hypertensive patients has a good effect on controlling blood pressure level and improving lifestyle.
