腹腔镜手术与开腹手术治疗宫外孕的临床效果对比

Comparison of clinical eff ects of laparoscopic surgery and open surgery on ectopic pregnancy

目的探讨腹腔镜手术与开腹手术治疗宫外孕的临床效果。方法选取50例宫外孕患者,随机分为对照组与研究组,每组25例。对照组患者应用开腹手术治疗,研究组患者应用腹腔镜手术治疗,对比两组术后恢复情况、并发症发生情况、疼痛评分及各项实验指标。结果研究组患者术中出血量显著少于对照组,手术时间、肛门排气时间、术后腹痛消失时间、留存尿管时间及住院时间均显著短于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为8.00%,显著低于对照组的36.00%,差异有统计学意义(P<0.05)。研究组患者疼痛评分为(1.25±0.58)分,显著低于对照组的(2.34±0.67)分,差异有统计学意义(P<0.05)。两组患者孕酮、β-人绒毛促性腺激素水平对比,差异无统计学意义(P>0.05),研究组血浆-D聚体、血管内皮生长因子水平显著低于对照组,差异有统计学意义(P<0.05)。结论临床治疗宫外孕应用腹腔镜手术治疗效果明显优于开腹手术,可降低并发症发生率,减少术中出血量,缩短住院时间,恢复激素水平,值得临床推广与应用。

Objective To investigate the clinical effect of laparoscopic surgery and open surgery in the treatment of ectopic pregnancy.Methods 50 cases of ectopic pregnancy were randomly divided into control group and study group,25 cases in each group.Patients in the control group were treated with open surgery,and patients in the study group were treated with laparoscopic surgery.Postoperative recovery,complications,pain score and various experimental indexes were compared between the two groups.Results The amount of intraoperative blood loss in the study group was signifi cantly less than that in the control group,and the operative time,anal exhaust time,postoperative abdominal pain disappearance time,urinary catheter retention time and hospital stay were signifi cantly shorter than those in the control group,with statistical signifi cance(P<0.05).The incidence of postoperative complications in the study group was 8.00%,significantly lower than that in the control group(36.00%),and the difference was statistically signifi cant(P<0.05).The pain score of the study group was(1.25±0.58),which was signifi cantly lower than that of the control group(2.34±0.67),and the difference was statistically signifi cant(P<0.05).There were no signifi cant differences in the levels of progesterone andβ-human villous gonadotropin between 2 groups(P>0.05),while the levels of plasma-D polymer and vascular endothelial growth factor in the study group were signifi cantly lower than those in the control group,with statistical signifi cance(P<0.05).Conclusion Clinical treatment of ectopic pregnancy using laparoscopic surgery is better than open surgery,can reduce the incidence of complications,reduce intraoperative blood loss,shorten the length of hospital stay,restore hormone levels,worthy of clinical promotion and application.
