摘要
目的探讨泪液总抗氧化能力、谷胱甘肽过氧化物酶水平与单纯疱疹病毒性角膜炎患者抗病毒药物治疗效果的相关性。方法对85例单纯疱疹病毒性角膜炎患者进行抗病毒药物治疗,根据治疗效果分为有效组及无效组,比较两组一般资料及泪液总抗氧化能力、谷胱甘肽过氧化物酶水平,分析单纯疱疹病毒性角膜炎患者泪液总抗氧化能力水平与谷胱甘肽过氧化物酶水平的相关性,对影响单纯疱疹病毒性角膜炎患者治疗效果的因素进行多元Logistic回归分析,分析总抗氧化能力、谷胱甘肽过氧化物酶水平对单纯疱疹病毒性角膜炎患者抗病毒药物治疗无效风险的预测效能。结果无效组泪液总抗氧化能力、谷胱甘肽过氧化物酶水平显著低于有效组(P<0.01)。单纯疱疹病毒性角膜炎患者泪液总抗氧化能力与谷胱甘肽过氧化物酶水平呈显著正相关(P<0.01)。泪液总抗氧化能力、谷胱甘肽过氧化物酶水平低表达可能是单纯疱疹病毒性角膜炎患者抗病毒治疗无效的影响因素(P<0.01)。泪液总抗氧化能力、谷胱甘肽过氧化物酶水平单独及联合预测单纯疱疹病毒性角膜炎患者抗病毒药物治疗无效风险的曲线下面积均>0.80,预测价值较理想,且联合预测效能最好。结论单纯疱疹病毒性角膜炎患者抗病毒药物治疗无效可能与泪液总抗氧化能力、谷胱甘肽过氧化物酶水平低表达有关,临床可利用其预测治疗无效风险,指导早期治疗方案合理调整。
Objective To observe the correlation between the expression levels of total antioxidant capacity(TAC)and glutathione peroxidase(GSH-Px)in tears and the therapeutic effect of antiviral drugs in patients with herpes simplex keratitis.Methods A total of 85 HSK patients treated with antiviral drugs were divided into an effective group and an ineffective group.The general data of the two groups were investigated,and the TAC and GSH-Px levels of tears were determined.Analyze the correlation between tear TAC level and GSH PX level in patients with HSK.Multivariate logistic regression analysis was used to analyze the factors influencing the therapeutic effect of HSK patients.Analyze the predictive efficacy of TAC and GSH-Px levels on the risk of ineffective antiviral therapy in HSK patients.Results The TAC and GSH-Px levels of tears in the ineffective group were significantly lower than those in the effective group(P<0.01).There was a significant positive correlation between tear TAC and GSH-Px levels in HSK patients(P<0.01).The low expression of TAC and GSH-Px in tears may be the influencing factors of the ineffectiveness of antiviral therapy in HSK patients(P<0.01).The TAC and GSH-Px levels in tears alone and in combination predicted the AUC of the risk of ineffective antiviral treatment in HSK patients were both>0.80.The predictive value was ideal,and the combined predictive performance was the best.Conclusions The ineffectiveness of antiviral drug treatment in HSK patients may be related to the low expression of TAC and GSH-Px in tears.Clinically,it may be considered to predict the risk of treatment ineffectiveness and guide the reasonable adjustment of early treatment plans.
