丙泊酚复合盐酸纳布啡或舒芬太尼对无痛纤维支气管镜检查患者麻醉质量及舒适满意度的影响 被引量:8

摘要 目的探讨丙泊酚复合盐酸纳布啡或舒芬太尼对无痛纤维支气管镜检查患者麻醉质量及舒适满意度的影响。方法将120例无痛纤维支气管镜检查患者按照随机数字表法分为两组,各60例。A组给予丙泊酚复合盐酸纳布啡,B组给予丙泊酚复合舒芬太尼。比较各时点两组舒张压、收缩压、心率水平。比较两组苏醒时间、丙泊酚用量、丙泊酚注射痛发生率、呛咳发生率及发生时间、不良反应发生率、舒适满意度。结果各时点两组舒张压、收缩压、心率水平比较差异均无统计学意义(P>0.05);诱导后、插镜时、插镜后5 min两组收缩压水平均显著低于麻醉前(P<0.01);诱导后A组舒张压水平显著低于麻醉前(P<0.01),诱导后、插镜时B组舒张压水平显著低于麻醉前(P<0.05或0.01);诱导后、插镜时、插镜后5 min、术毕两组心率水平均较麻醉前显著降低(P<0.01)。A组苏醒时间显著短于B组(P<0.01),丙泊酚用量显著少于B组(P<0.01)。A组丙泊酚注射痛发生率显著低于B组(P<0.05),呛咳发生率显著低于B组(P<0.05),呛咳发生时间显著晚于B组(P<0.01)。A组不良反应发生率显著低于B组(P<0.01),舒适满意率显著高于B组(P<0.05)。结论丙泊酚复合盐酸纳布啡能有效降低无痛纤维支气管镜检查患者丙泊酚注射痛、呛咳及不良反应发生率,提高麻醉质量,提升舒适满意度。 Objective To investigate the effect of propofol combined with nalbuphine hydrochloride or sufentanil on the anesthesia quality and comfort satisfaction of patients undergoing painless fiberoptic bronchoscopy.Methods A total of 120 patients with painless fiberoptic bronchoscopy were divided into two groups according to the random number table method,with 60 cases in each group.Group A was given propofol combined with nalbuphine hydrochloride,and group B was given propofol combined with sufentanil.Compare the diastolic blood pressure(DBP),systolic blood pressure(SBP),and heart rate(HR)levels of the two groups at each time point.The recovery time,the dosage of propofol,the incidence of propofol injection pain,the incidence and time of coughing,the incidence of adverse reactions,and the satisfaction degree of comfort were compared between the two groups.Results There was no statistically significant difference in the levels of DBP,SBP,and HR between the two groups at each time point(P>0.05).SBP levels after induction,during and 5 min after implantation in both groups were significantly lower than those before anesthesia(P<0.01).After induction,the DBP level of group A was significantly lower than that before anesthesia(P<0.01),and the DBP level of group B after induction and during implantation was significantly lower than that before anesthesia(P<0.05 or 0.01).HR levels after induction,during and 5 min after implantation and at the end of operation of the two groups were significantly lower than those before anesthesia(P<0.01).The recovery time of group A was significantly shorter than that of group B(P<0.01),and the dosage of propofol was significantly less than that of group B(P<0.01).The incidence of propofol injection pain in group A was significantly lower than that in group B(P<0.05),the incidence of cough was significantly lower than that in group B(P<0.05),and the occurrence time of cough was significantly later than that in group B(P<0.01).The incidence of adverse reactions in group A was significantly lower than that in group B(P<0.01),and the comfort satisfaction rate was significantly higher than that in group B(P<0.05).Conclusions Propofol combined with nalbuphine hydrochloride can effectively reduce the incidence of propofol injection pain,cough and adverse reactions in patients undergoing painless fiberoptic bronchoscopy,improve the quality of anesthesia and comfort satisfaction.
