以故事理论为基础的护理干预对系统性红斑狼疮患者应对方式及自护能力的影响 被引量:6

摘要 目的探讨以故事理论为基础的护理干预对系统性红斑狼疮患者应对方式及自护能力的影响。方法将92例系统性红斑狼疮患者按照随机数字表法分为研究组与对照组,各46例。两组均给予常规护理干预,研究组在此基础上给予以故事理论为基础的护理干预。干预前后采用焦虑自评量表、抑郁自评量表评定两组焦虑、抑郁程度,医学应对问卷评定两组应对方式,Morisky用药依从性问卷评定两组服药依从性,系统性红斑狼疮自护能力量表评定两组自护能力,世界卫生组织生存质量测定量表简表评定两组生存质量,纽卡斯尔护理满意度量表评定两组护理质量。结果干预后两组焦虑自评量表及抑郁自评量表评分显著低于干预前(P<0.01),研究组显著低于对照组(P<0.01);研究组医学应对问卷面对评分显著高于干预前(P<0.01),回避、屈服评分显著低于干预前(P<0.01),对照组面对评分显著高于干预前(P<0.01),回避评分显著低于干预前(P<0.01),研究组较对照组变化更显著(P<0.01)。干预后两组系统性红斑狼疮自护能力量表各维度评分及世界卫生组织生存质量测定量表简表评分均显著高于干预前(P<0.01),研究组显著高于对照组(P<0.01)。研究组低服药依从性显著优于对照组(P<0.05),护理总满意率显著高于对照组(P<0.05)。结论以故事理论为基础的护理干预可显著缓解系统性红斑狼疮患者的负性情绪,提高自护能力及服药依从性,改善应对方式及生活质量,患者护理满意度较高,值得推广应用。 Objective To explore the effect of nursing intervention based on story theory on coping style and self-care ability of patients with systemic lupus erythematosus.Methods A total of 92 patients with systemic lupus erythematosus were divided into a study group and a control group according to the random number table method,each with 46 cases.Both groups were given routine nursing intervention,and the study group was given nursing intervention based on story theory on this basis.Before and after the intervention,the anxiety and depression levels of the two groups were assessed with the self-rating anxiety scale(SAS)and self-rating depression scale(SDS),the medical coping modes questionnaire(MCMQ)was used to assess the coping styles of the two groups,and the Morisky medication adherence scale(MMAS-8)was used to assess the medication compliance of the two groups and the self-care ability scale for systemic lupus erythematosus was used to assess the self-care ability of the two groups.The World Health Organization quality of life brief version(WHOQOL-BREF)was used to assess the quality of life of the two groups,and the Newcastle satisfaction with nursing scales(NSNS)was used to assess the quality of care of the two groups.Results After the intervention,the SAS and SDS scores of the two groups were sign-ificantly lower than those before the intervention(P<0.01),and the study group was significantly lower than the control group(P<0.01).The MCMQ face score of the study group was significantly higher than that before the intervention(P<0.01),and the avoidance and yield scores were significantly lower than those before the intervention(P<0.01).The face score of the control group was significantly higher than that before the intervention(P<0.01),and the avoidance score was significantly lower than that before the intervention(P<0.01).The study group changed more significantly than the control group(P<0.01).After the intervention,the scores of self-care ability scale for systemic lupus erythematosus in all dimensions and the WHOQOL-BREF scores of the two groups were significantly higher than those before the intervention(P<0.01),the study group was significantly higher than the control group(P<0.01).The medication compliance of the study group was significantly better than that of the control group(P<0.05),and the nursing satisfaction rate was significantly higher than that of the control group(P<0.05).Conclusions Nursing intervention based on story theory can significantly alleviate the negative emotions of patients with systemic lupus erythematosus,improve self-care ability and medication compliance,improve coping styles and quality of life,and patient care satisfaction is high,which is worthy of promotion and application.
