针康法对卒中后抑郁患者认知及神经功能恢复的影响

Effects of Acupuncture-Rehabilitation Therapy on Cognitive and Neurological Recovery in Patients with Post-Stroke Depression

目的:研究针康法对卒中后抑郁患者(PSD)运动障碍、日常生活活动能力、认知障碍、负性情绪的疗效。方法:将49例缺血性脑卒中后抑郁患者随机分为治疗组(25例)和对照组(24例),对照组采用音乐治疗,治疗组在音乐治疗同时加用头穴丛刺治疗,均治疗12周。于干预前和干预12周结束时采用Fugl-Meyer运动功能评分量表(FMA)、Barthel指数(BI)简易精神状态评价量表(MMSE)、抑郁自评量表(SDS)、脑功能分析仪评价运动功能、日常生活能力、认知功能、抑郁状况、脑内5-羟色胺(5-HT)水平。结果:临床治疗后患者FMA评分、BI指数、MMSE、5-HT水平均较治疗前显著增高(P<0.05),SDS评分显著低于患者治疗前评分(P<0.05)。结论:针康法可以提高脑卒中患者的运动功能、日常生活能力、认知功能,改善抑郁症状和5-HT水平,对于脑卒中后抑郁患者的康复有一定的积极影响。

Objective:To study the effect of acupuncture-rehabilitation therapy on dyskinesia,activities of daily living(ADL),cognitive impairment and negative emotions in the treatment of post-stroke depression(PSD).Methods:49 patients with PSD were randomly divided into the treatment group(n=25)and the control group(n=24).The control group was treated with music therapy;on which basis,the treatment group was also treated with scalp cluster needling.Both groups were treated for 12 weeks.Before and at the end of 12 weeks of intervention,the scores of Fugl-Meyer Assessment of Motor Function scale(FMA),Barthel index(BI),Mini-mental Status Examination(MMSE)and Self-rating Depression Scale(SDS),as well as the brain function analysis instrument were used to evaluate motor ability,ADL,cognitive function,depression state.and the level of 5-HT in brain.Results:The scores of FMA,BI and MMSE,as well as the level of 5-HT were significantly higher,and the score of SDS was significantly lower after the treatment than those before the treatment in the two group(P<0.05);of which the improvements in the treatment group were more significant(P<0.05).Conclusion:Acupuncture-rehabilitation therapy can improve motor function,ADL,cognitive function,depression symptoms and the level of 5-HT in the treatment of PSD.
