气管支气管低度恶性肿瘤CT表现 被引量:3

摘要 目的:通过探讨气管支气管低度恶性肿瘤CT表现以提高对该病诊断准确性。方法:回顾性分析经病理证实气管支气管低度恶性肿瘤80例CT表现及临床特征,总结其规律。结果:80例中腺样囊性癌37例,黏液表皮样癌26例及类癌17例。发病部位:腺样囊性癌发生于气管24例,主支气管10例,叶支气管1例,段支气管2例;黏液表皮样癌发生于气管的7例,主支气管7例,叶支气管2例,段支气管10例;类癌发生于主支气管9例,段支气管8例。病灶形态:腺样囊性癌显示结节型13例,肿块型6例,管壁增厚型18例;黏液表皮样癌显示结节型23例,肿块型3例;类癌显示结节型8例,肿块型9例。强化模式:腺样囊性癌△CT值(平扫期与静脉期差值)平均值(25.30±12.39)HU,黏液表皮样癌△CT值平均值(61.96±29.93)HU,类癌△CT值平均值(62.06±31.40)HU,ΔCT值存在统计学差异(P=0.000,P<0.001)。结论:气管支气管3种低度恶性肿瘤CT上可通过观察发病部位、病灶形态、强化模式等情况进行鉴别诊断。 Objective:In order to improve the diagnostic accuracy of low-grade malignant tumor of traches and bronchus by discussing the CT findings.Methods:The CT findings and clinical features of 80 cases of low-grade tracheal and bronchial malignant tumor confirmed by pathology were retrospectively analyzed and their regularities were summarized.Results:Among the 80 cases,37 were adenoid cystic carcinoma,26 were mucoepidermoid carcinoma and 17 were carcinoid.Lesion location:Adenoid cystic carcinoma occurred in trachea in 24 cases,main bronchus in 10 cases,lobar bronchus in 1 case,segmental bronchus in 2 cases.Mucoepidermoid carcinoma occurred in trachea in 7 cases,main bronchus in 7 cases,lobar bronchus in 2 cases,segmental bronchus in 10 cases.Carcinoid occurred in the main bronchus in 9 cases and segmental bronchus in 8 cases.Lesion morphology:Adenoid cystic carcinoma showed nodular type(n=13),mass type(n=6)and wall thickening type(n=18).Mucoepidermoid carcinoma showed nodular type in 23 cases and mass type in 3 cases.Carcinoid showed nodular type(n=8)and mass type(n=9).Enhancement:the average△CT value of adenoid cystic carcinoma,mucoepidermoid carcinoma and carcinoid were(25.30±12.39)HU,(61.96±29.93)HU and(62.06±31.40)HU,respectively,which had statistically significance(P=0.000,P<0.001).Conclusion:Three kinds of primary low-grade tracheobronchial malignant tumors can be distinguished by observing the location,shape and enhancement degree of the disease on CT.
