rb-bFGF联合利多卡因治疗儿童复发性口腔溃疡的效果及对患儿Treg细胞的影响 被引量:10

The effect of rb-bFGF combined with lidocaine in the treatment of recurrent oral ulcers in children and its influence on Treg cells in children

摘要 目的观察重组牛碱性成纤维细胞生长因子(rb-bFGF)联合利多卡因治疗儿童复发性口腔溃疡的效果及对CD4^(+)、CD25^(+)、Foxp3^(+)、Treg细胞的影响。方法选取2019年1月—2020年6月湖南中医药大学第一附属医院口腔科诊治复发性口腔溃疡患儿156例,随机数字表法分为利多卡因组和联合用药组,各78例。利多卡因组采用利多卡因治疗,联合用药组在其基础上增加rb-bFGF治疗,2组均治疗7 d。比较2组患儿治疗效果及临床症状缓解时间,溃疡面积、个数,VAS评分,Treg细胞因子、炎性因子水平及不良反应。结果联合用药组治疗总有效率为98.72%,高于利多卡因组的89.74%(χ2/P=5.778/0.016);疼痛消失、溃疡愈合、进食改善时间明显短于利多卡因组(t/P=9.078/0.000、8.721/0.000、8.104/0.000);治疗7 d后,联合用药组溃疡面积小于利多卡因组(t/P=13.320/0.000),溃疡个数少于利多卡因组(t/P=12.840/0.000),VAS评分、TNF-α、IL-8水平低于利多卡因组(t/P=16.780/0.000、6.237/0.000、5.274/0.000);联合用药组Treg细胞因子CD3+、CD4+、CD8+、NKT、IL-2、IL-4水平高于利多卡因组(t/P=2.892/0.004、3.095/0.000、3.654/0.000、6.499/0.000、2.896/0.000、5.606/0.000);联合用药组不良反应发生率高于利多卡因组,但差异无统计学意义(χ2/P=1.346/0.246)。结论重组牛碱性成纤维细胞生长因子联合利多卡因治疗儿童复发性口腔溃疡可有效促进溃疡恢复,缓解临床症状,减轻疼痛感和机体炎性反应,增强免疫功能,效果显著。 Objective To observe the effect of recombinant bovine basic fibroblast growth factor(rb-bFGF)combined with lidocaine in the treatment of recurrent oral ulcers in children and the effect on CD4^(+),CD25^(+),Foxp3^(+)and Treg cells.Methods From January 2019 to June 2020,156 children with recurrent oral ulcers were diagnosed and treated in the Department of Stomatology,the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine.The random number table was divided into lidocaine group and combination medication group,with 78 cases in each group.The lidocaine group was treated with lidocaine,and the combination group was treated with rb-bFGF.Both groups were treated for 7 days.The treatment effect and clinical symptom relief time,ulcer area,number,VAS score,Treg cytokine,inflammatory factor levels and adverse reactions were compared between the two groups of children.Results The total effective rate of treatment in the combination group was 98.72%,which was higher than 89.74%in the lidocaine group(χ2/P=5.778/0.016);the pain disappeared,the ulcer healed,and the eating improvement time was significantly shorter than that in the lidocaine group t/P=9.078/0.000,8.721/0.000,8.104/0.000);after 7 days of treatment,the area of ulcers in the combination group was smaller than that in the lidocaine group(t/P=13.320/0.000),and the number of ulcers was less than that in the lidocaine group(t/P=12.840/0.000),VAS score,TNF-α,IL-8 levels were lower than lidocaine group(t/P=16.780/0.000,6.237/0.000,5.274/0.000);Treg cytokines CD3+,CD4+,The levels of CD8+,NKT,IL-2,and IL-4 were higher than those in the lidocaine group(t/P=2.892/0.004,3.095/0.000,3.654/0.000,6.499/0.000,2.896/0.000,5.606/0.000);the combination group was poor The reaction rate was higher than that in the lidocaine group,but the difference was not statistically significant(χ2/P=1.346/0.246).Conclusion Recombinant bovine basic fibroblast growth factor combined with lidocaine in the treatment of recurrent oral ulcers in children can effectively promote the recovery of ulcers,relieve clinical symptoms,relieve pain and body inflammatory reactions,and enhance immune function with significant effects.
