拇外翻与扁平足影像学相关性研究

Correlation research of imaging parameters between hallux valgus and pes planus

目的分析拇外翻与扁平足之间的影像学相关性。方法回顾性分析北部战区总医院自2017年1月至2019年1月收治的131例足踝部疾病患者的临床资料。131例患者中,拇外翻61例(拇外翻组),非拇外翻70例(非拇外翻组)。所有患者均接受负重下患足正、侧位X线检查,采用PACS系统测量与扁平足相关的(正位)距骨第1跖骨角(TMA),距舟覆盖角(TNCA),距骨跟骨角(Kite角),(侧位)距骨第1跖骨角(Meary角),跟骨倾斜角(Pitch角)及拇外翻角(HVA),第1、2跖骨间角(IMA)。比较拇外翻组和非拇外翻组HVA、IMA、TMA、TNCA、Kite角、Meary角、Pitch角的差异,并采用Pearson法或Spearman法分析相关性。结果拇外翻组HVA、IMA、TMA、TNCA、Kite角、Meary角大于非拇外翻组,Pitch角小于非拇外翻组,差异有统计学意义(P<0.05)。在拇外翻组中,Meary角、Pitch角、Kite角、TMA、TNCA正常的比例明显低于非拇外翻组,差异有统计学意义(P<0.05)。HVA与IMA、Kite角、TMA、TNCA、Meary角呈正相关,与Pitch角呈负相关(P<0.05);IMA与Kite角、TMA、TNCA、Meary角呈正相关,与Pitch角呈负相关(P<0.05)。结论拇外翻与扁平足在影像学上具有相关性。

Objective To analyze the imaging correlation between hallux valgus and pes planus.Methods A retrospective study was performed on 131 cases of patients with ankle disease who were admitted from January 2017 to January 2019.The patients were divided into hallux valgus group(n=61)and non-hallux valgus group(n=70).PACS was used to measure hallux valgus angle(HVA),intermetatarsal angle(IMA),talus-first metatarsal angle(TMA),talonavicular coverage angle(TNCA),Kite angle, Meary angle and Pitch angle.All the imaging parameters were compared and the correlation between variables was analyzed.Results HVA,IMA,TMA,TNCA,Kite angle and Meary angle in hallux valgus group were significantly higher than those in non-hallux valgus group, while Pitch angle was significantly lower than that in non-hallux valgus group(P<0.05).The proportion of normal values of Meary angle, Pitch angle, Kite angle, TMA and TNCA in hallux valgus group were significantly lower than those in non-hallux valgus group(P<0.05).HVA was positively correlated with IMA,Kite angle, TMA,TNCA and Meary angle, negatively correlated with Pitch angle(P<0.05).IMA was positively correlated with Kite angle, TMA,TNCA and Meary angle, negatively correlated with Pitch angle(P<0.05).Conclusion There is a high correlation of imaging parameters between hallux valgus and pes planus.
