紫杉醇脂质体联合奥沙利铂和替吉奥治疗晚期胃癌的疗效观察

Efficacy of paclitaxel liposomes combined with oxaliplatin and tigio for advanced gastric cancer

目的探讨紫杉醇脂质联合奥沙利铂和替吉奥治疗晚期胃癌的疗效。方法选取2018年1月至2020年6月间辽宁省营口市中心医院收治的120例晚期胃癌患者,采用随机数表法分为观察组和对照组,每组60例。观察组患者采用紫杉醇脂质体联合奥沙利铂和替吉奥治疗,对照组患者采用多西他赛联合奥沙利铂和替吉奥治疗,比较两组患者治疗效果、免疫情况、生存情况和不良反应。结果观察组患者治疗总有效率为66.7%,优于对照组的40.0%,差异有统计学意义(P<0.05)。治疗前,两组患者CD3^(+)、CD4^(+)、CD8^(+)和CD4^(+)/CD8^(+)组间比较,差异均无统计学意义(P>0.05)。治疗后,两组患者CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)均优于治疗前,差异均有统计学意义(均P<0.05)。观察组患者中位生存时间为19个月,对照组为18个月,差异无统计学差异(P>0.05)。观察组中性粒细胞减少和贫血总发生率均优于对照组,观察组中性粒细胞减少、恶心/呕吐、口腔黏膜炎和手足综合征发生程度均优于对照组,差异均有统计学意义(均P<0.05)。结论紫杉醇脂质体联合奥沙利铂和替吉奥治疗晚期胃癌,能改善患者的治疗效果和免疫能力,降低中性粒细胞减少和贫血发生率,安全性高,值得推广应用。

Objective To explore the efficacy of paclitaxel lipid combined with oxaliplatin and tigio in the treatment of advanced cancer.Methods A total of 120 patients with advanced gastric cancer diagnosed at Yingkou Central Hospital from January 2018 to June 2020 were selected.They were divided into an observation group and a control group using random number table with 60 patients each.The observation group received paclitaxel liposomes combined with oxaliplatin and tigeo,and the control group received docetaxel combined with oxaliplatin and tigio.Treatment efficacy,immune status,survival and adverse reactions were compared between the two groups.Results The overall efficacy was 66.7% in the observation group,which was better than 40.0% of the control group(P<0.05).There was no significant difference in lymphocyte including CD3^(+),CD4^(+),CD8^(+) and CD4^(+)/CD8^(+) between the two groups before the treatment(P>0.05).After the treatment,levels of CD3^(+),CD4^(+) and CD4^(+)/CD8^(+)were improved in the two groups compared with those before the treatment(all P<0.05).There was no statistical difference in median survival between the observation group(19 months)and the control group(18 months)(P>0.05).The overall incidences of neutropenia and anemia was less in the observation group than in the control group(P<0.05).The overall incidence of neutropenia,nausea/vomiting,oral mucositis and hand-foot syndrome was less in the observation group than in the control group(P<0.05).Conclusion Paclitaxel liposomes combined with oxaliplatin and tigio in the treatment of advanced cancer can improve the treatment efficacy and immunity,reduce the incidence of neutrophils and anemia with good safety.It is worthy of promotion.
