三级甲等综合医院住院病人护理难度评价指标体系的构建研究

Study on construction of evaluation index system for difficulty of inpatient nursing in third grade A general hospitals

目的:构建科学且适用于三级甲等综合医院住院病人的护理难度评价指标体系,为合理配置护理人力资源及制定科学的护理人员薪酬制度提供依据。方法:基于文献回顾与质性访谈初步拟定评价指标草案,采用德尔菲法对28名专家进行两轮函询最终确立评价指标体系。结果:两轮专家咨询后,专家积极系数为92.59%,专家权威系数为0.874,肯德尔系数为0.195~0.234(均P<0.05),最终确立了病人一般状况、病人社会支持状况、病人疾病护理难度、护理实践环境支持4个一级评价指标以及17个二级评价指标。结论:构建的三级甲等综合医院住院病人护理难度评价指标体系具有一定的科学性、可靠性和适用性,可在护理人力资源配置及护理人员绩效考核中加以运用。

Objective:To construct a scientific evaluation index system for the nursing difficulty of the inpatients in third grade A general hospitals,and to provide a basis for the rational allocation of nursing human resources and the establishment of a scientific compensation system for nursing staff.Methods:Based on literature review and qualitative interview,a preliminary draft of evaluation indicators was developed.The Delphi method was used to conduct two rounds of letter consultation among 28 experts to finally establish the evaluation index system.Results:After two rounds of expert consultation,the expert positive coefficient,expert authority coefficient and Kendall coefficient were 92.59%,0.874 and 0.195~0.234 respectively(all P<0.05).Finally,four primary evaluation indexes including the general condition of patients,the social support of patients,the difficulty of patient disease nursing,and the support of nursing practice environment,and 17 secondary evaluation indexes were established.Conclusions:The evaluation index system of the nursing difficulty for the inpatients in third grade A general hospitals had certain scientific nature,reliability and applicability,and can be used in the allocation of nursing human resources and the performance assessment of nursing staff.
