超声联合X线、CT诊断肠梗阻的临床应用价值

Clinical value of ultrasound combined with X-ray and CT in the diagnosis of intestinal obstruction

目的探讨X线、CT及超声三者联合应用在肠梗阻诊断中的应用价值。方法收集手术确诊的肠梗阻患者80例,均于治疗前行X线平片、CT及超声检查,比较三种检查方法单独应用以及联合应用的诊断效能。结果X线、CT及超声诊断肠梗阻的准确率、对梗阻部位诊断的准确率以及对梗阻原因诊断的准确率分别为75.0%、56.3%、41.2%;87.5%、75.0%、60%及77.5%、60.0%、45.0%;三者联合应用诊断肠梗阻的准确率、对梗阻部位诊断的准确率以及对梗阻原因诊断的准确率为91.3%、80.0%、66.3%。X线及超声两种检查方法间诊断效能无明显差异,CT诊断效能高于X线及超声,三者联合应用的诊断效能高于单独应用一种方法,差异具有统计学意义(P<0.05)。结论X线及超声对肠梗阻的诊断效能没有明显差异,CT诊断效能高于X线及超声,而三者联合应用可明显提高诊断率。

Objective To investigate the application value of X-ray,CT and ultrasound in the diagnosis of intestinal obstruction.Methods Eighty patients with intestinal obstruction diagnosed by operation were collected.X-ray plain film,CT and ultrasound examination were performed before treatment.The diagnostic efficacy of three examination methods alone and combined application were compared.Results The accuracy of X-ray,CT and ultrasound in the diagnosis of intestinal obstruction,the location of obstruction and the cause of obstruction were 75.0%,56.3%and 41.2%respectively;87.5%,75.0%,60%and 77.5%,60.0%,45.0%;the combined application of the three diagnostic accuracy of intestinal obstruction,the accuracy of the diagnosis of the location of obstruction and the accuracy of the diagnosis of the cause of obstruction were 91.3%,80.0%,66.3%.There was no significant difference in the diagnostic efficiency between X-ray and ultrasound.The diagnostic efficiency of CT was higher than that of X-ray and ultrasound.The diagnostic efficiency of combination of the three methods was higher than that of single method,and the difference was statistically significant(P<0.05).Conclusion There is no significant difference between X-ray and ultrasound in the diagnosis of intestinal obstruction.The diagnostic efficiency of CT is higher than that of X-ray and ultrasound.
