摘要
目的:分析并探讨医院机会性筛查女性中宫颈人乳头瘤病毒(human papillomavirus,HPV)多重感染状况,以帮助开发HPV疫苗及今后疫苗接种效果的长期监测.方法:回顾性收集2010年1月至2018年12月期间64911名在本院机会性筛查女性中HPV多重感染及亚型分布结果.结果:64911名接受了分型检测的女性中,HPV总感染率为29.10%,其中多重感染共5666例,占阳性感染人数中的29.99%,排在前5位的亚型依次为:HPV52、16、58、81、56;多重感染中以二重感染为主,共3895例,其中有235种不同亚型组合,最常见的组合依次为:HPV16+52、16+58、52+81、52+58、52+53;不同年份单一感染和多重感染的差异有统计学意义(P<0.001);不同年龄段HPV多重感染的差异有统计学意义(P<0.001),HPV多重感染率在<30岁和≥60岁女性中最高.结论:HPV多重感染主要为二重感染,其中52型、16型检出率较高.临床上对HPV多重感染现状进行调查分析,能够为HPV疫苗的开发、应用及接种效果提供基线数据.
Objective:To analyze the prevalence of multiple cervical human papillomavirus(HPV)infection among women undergoing opportunistic screening in our hospital,which helps to develop HPV vaccine and long-term monitoring of future vaccination effects.Methods:Results of HPV multiple infection and subtype distribution in 64,911 women undergoing opportunistic screening in our hospital from January 2010 to December 2018 were collected retrospectively.Results:A-mong the 64,911 women who underwent HPV typing test,the total infection rate of HPV was 29.10%.Among them,5,666 cases were multiple infection,accounting for 29.99%of the positive cases.The top five subtypes were HPV52,16,58,81 and 56.In total,3,895 cases of multiple infection were double infection,of which there were 235 different combinations,and the most common combinations were HPV 16+52,16+58,52+81,52+58 and 52+53,respectively.There were significant differences in single infection and multiple infection rates among different years(P<0.001),and there were significant differences in HPV multiple infection rates among different age groups(P<0.001).The multiple infection rate was the highest in women under 30 years old and women aged 60 and older.Conclusion:HPV multiple infections are mainly double infection,in which the detection rates of type 52 and 16 are comparatively high.Clinical investigations and analyses of the current situation of HPV multiple infections can provide baseline data for the development,application and effects of HPV vaccines.
