摘要 目的:探讨血浆同型半胱氨酸(HCY)水平、血浆N-末端脑利钠肽前体(NT-proBNP)水平指导原发性高血压患者心血管危险分层价值研究。方法:回顾性分析2019年5月至2020年3月期间,百色市右江区人民医院接诊的90例原发性高血压患者进行危险分层,分为中危组、高危组以及极高危组,另选取50例体检健康志愿者作为健康组,对比分析健康组与高血压组患者的HCY水平、NT-proBNP水平,并分析HCY水平、NT-proBNP水平与患者危险分层之间关系。结果:高血压组患者的血压情况、高密度脂蛋白以及BMI等水平显著高于健康组(P<0.05);依照高血压危险程度分级标准将患者分为三组,其中极高危组患者的HCY水平值、NT-proBNP水平值高于高危组,高危组患者的HCY水平、NT-proBNP水平高于中危组(P<0.05);HCY水平值随着高血压伴随颈动脉内膜中层厚度(IMT)增加而上升(P<0.05)。结论:原发性高血压患者的HCY、NT-proBNP水平与危险程度相关性,HCY、NT-proBNP水平变化对与原发性高血压患者病情判断具有指导意义。 Objective:To investigate the value of plasma homocysteine(Hcy)level and plasma N-terminal brain natriuretic peptide precursor(NT-proBNP)level in guiding cardiovascular risk stratification in patients with essential hypertension.Methods:A retrospective analysis in May 2019 to March 2020,People's Hospital of Youjiang District departments of 90 patients with primary hypertension risk stratification,divided into moderate group,high-risk groups and extremely high-risk group,another 50 cases selected medical healthy volunteers as health group,comparison and analysis of health group and HCY levels in patients with hypertension group,NT-proBNP level,and analyze the HCY levels,NT-proBNP level and the relationship between the patient risk stratification.Results:The levels of blood pressure,high density lipoprotein and BMI in hypertension group were significantly higher than those in healthy group(P<0.05).The patients were divided into three groups according to the classification standard of hypertension risk degree.The Hcy level and NT-proBNP level of patients in the extremely high-risk group were higher than those in the high-risk group,and the Hcy level and NT-proBNP level of patients in the high-risk group were higher than those in the medium-risk group(P<0.05).The level of Hcy increased with hypertension and the increase of carotid intima media thickness(IMT)(P<0.05).Conclusion:The level of Hcy and NT-proBNP in patients with essential hypertension is correlated with the risk degree,and the change of Hcy and NT-proBNP level has guiding significance for the judgment of the condition of patients with essential hypertension.
