肺功能检查在社区慢性阻塞性肺疾病早期诊断中的应用

Application of Pulmonary Function Test in Early Diagnosis of Chronic Obstructive Pulmonary Disease in Community

目的:探讨便携式肺功能检查在社区COPD早期诊断中的应用价值。方法:以100例健康体检者为健康组、100例吸烟无症状者为吸烟无症状组、100例吸烟有症状者为吸烟有症状组,对比三组各项便携式肺功能检查指标间差异,并分析COPD严重程度与吸烟指数间的相关性。结果:三组间FVC比较,差异未见统计学意义(P>0.05),而FEV1%、FEV1/FVC%、PEF%、FEF25%、FEF50%、FEF75%及MMEF%水平比较,差异具统计学意义(P<0.05);吸烟有症状组FEV1%、FEV1/FVC%、PEF%、FEF25%、FEF50%、FEF75%及MMEF%水平低于吸烟无症状组及健康组,吸烟无症状组FEV1%、FEV1/FVC%、PEF%、FEF25%、FEF50%、FEF75%及MMEF%水平低于健康组(P<0.05)。同时,在COPD患者中,FEV1%与吸烟指数呈显著的负向直线相关性(P<0.05)。结论:便携式肺功能仪检查有助于早期发现社区具有高危因素的COPD患者,减少漏诊,值得在社区医疗机构推广使用。

Objective:To explore the value of portable pulmonary function test in the early diagnosis of community COPD.Methods:100 healthy subjects were taken as the healthy group,100 asymptomatic smokers as the asymptomatic group,and 100 symptomatic smokers as the symptomatic group,the correlation between the severity of COPD and smoking index was analyzed.Results:There was no significant difference in FVC among the three groups(P>0.05),but significant difference in FEV1%,FEV1/FVC%,PEF%,FEF25%,FEF50%,FEF75% and MMEF%(P<0.05);FEV1%,FEV 1/FVC%,PEF%,FEF25%,FEF50%,FEF75% and MMEF% in symptomatic smoking group were lower than those in asymptomatic smoking group and healthy smoking group.FEV1%,FEV 1/FVC%,PEF%,FEF25%,FEF50%,FEF75%and MMEF%in asymptomatic smoking group were lower than those in healthy group(P<0.05).In COPD patients,there was a significant negative linear correlation between FEV1%and smoking index(P<0.05).Conclusion:The Portable Pulmonary function test is helpful to find the patients with COPD who have high risk factors in the community early,reduce the missed diagnosis,and it is worth to promote the use of community medical institutions.
