精神分裂症患者睡眠问题的潜在类别分析

Analysis of Potential Categories of Sleep Problems in Patients with Schizophrenia

目的探究精神分裂症患者睡眠问题的潜在类别及其与抑郁、焦虑及压力之间的关系。方法采用匹兹堡睡眠质量量表(PSQI)和抑郁-焦虑-压力量表(DASS-21)对湖南省第二人民医院精神病科的299例精神分裂症患者进行问卷调查,并对测验数据进行潜在剖面分析及差异性检验。结果精神分裂症患者的睡眠问题被划分为3个潜在类别,分别是睡眠良好组(63.5%)、睡眠障碍-高催眠药组(12.4%)和主观睡眠不良组(24.1%)。睡眠障碍-高催眠药组患者的抑郁、焦虑和压力水平均高于睡眠良好组和主观睡眠不良组,主观睡眠不良组患者的抑郁、焦虑和压力水平均高于睡眠良好组(P均<0.05)。结论精神分裂症患者的睡眠问题存在3个质性不同的潜在类别,不同潜在类别的抑郁、焦虑和压力水平也不同,可为精神分裂症患者睡眠问题的临床心理辅导提供相应理论指导。

Objective To explore the underlying categories of sleep problems in patients with schizophrenia and their relationship with depression,anxiety and stress.Methods The Pittsburgh sleep quality index(PSQI)and the depression-anxiety stress inventory(DASS-21)were used to investigate 299 patients with schizophrenia in the Department of Psychiatry of the Second People's Hospital of Hunan Province.The latent profile analysis and difference test were performed on the data.Results The sleep problems of patients with schizophrenia were divided into 3 latent categories:good sleep group(63.5%),sleep disorder-high hypnosis group(12.4%)and subjective poor sleep group(24.1%).Patients in the sleep disorder-high hypnosis group had higher levels of depression,anxiety,and stress than those in the good sleep group and the subjective poor sleep group;patients in the subjective poor sleep group had higher levels of depression,anxiety,and stress than those in the good sleep group(P all<0.05).Conclusion There are three potential categories of sleep problems in schizophrenic patients,and the levels of depression,anxiety and stress in different potential categories are also significantly different,which can provide corresponding theoretical guidance for clinical psychological counseling of sleep problems in schizophrenic patients.
