铝碳酸镁联合幽门螺杆菌根治对幽门螺杆菌感染并十二指肠溃疡患者胃窦炎症程度、炎症介质、胃肠激素水平的影响

Effect of hydrotalcite combined with Helicobacter pylori eradication on antral inflammation degree,inflammatory mediators and gastrointestinal hormone levels in patients with Helicobacter pylori infection and duodenal ulcer

目的分析铝碳酸镁联合幽门螺杆菌(Hp)根除治疗(根治)对Hp感染并十二指肠溃疡(DU)患者胃窦炎症程度、炎症介质、胃肠激素水平的影响。方法98例Hp感染并十二指肠溃疡患者,以双盲法分为对照组和实验组,各49例。对照组采取四联Hp根治,实验组在对照组的基础上增加铝碳酸镁治疗。比较两组治疗后的胃窦黏膜组织活动性炎症程度,治疗前后的炎症介质[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]及胃肠激素[血浆人胃泌素-17(Gas-17)、黏膜组织Gas-17]水平。结果实验组患者的胃窦黏膜组织活动性炎症程度优于对照组,差异有统计学意义(Z=2.206,P=0.027<0.05)。治疗后,实验组患者的TNF-α(38.42±10.01)pg/L、IL-6(41.05±11.98)pg/L明显低于对照组的(45.33±11.96)、(48.77±13.95)pg/L,差异具有统计学意义(P<0.05)。实验组患者的血浆Gas-17(90.08±22.52)ng/L、黏膜组织Gas-17(303.20±50.94)ng/L明显低于对照组的(99.79±24.56)、(329.57±55.12)ng/L,差异具有统计学意义(P<0.05)。结论Hp感染并十二指肠溃疡患者临床治疗中选择铝碳酸镁联合Hp根治不仅可以进一步改善患者胃窦炎症程度,而且有利于降低炎症介质水平,对其胃肠激素水平带来良好的调节作用。

Objective To analyze the effect of hydrotalcite combined with Helicobacter pylori(Hp)eradication on the degree of antral inflammation,inflammatory mediators and gastrointestinal hormone levels in patients with Helicobacter pylori infection and duodenal ulcer(DU).Methods A total of 98 patients with Hp infection and duodenal ulcer were divided into control group and experimental group according to double-blind method,with 49 cases in each group.The control group was treated with quadruple HP eradication therapy,and the experimental group was treated with hydrotalcite on the basis of the control group.The active inflammation degree of antral mucosa after treatment,inflammatory mediators[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]and gastrointestinal hormones[plasma human gastrin-17(Gas-17),mucosal tissue Gas-17]before and after treatment were compared between the two groups.Results The active inflammation degree of antral mucosa of the experimental group was better than that of the control group,and the difference was statistically significant(Z=2.206,P=0.027<0.05).After treatment,the TNF-α(38.42±10.01)pg/L and IL-6(41.05±11.98)pg/L of the experimental group were obviously lower than(45.33±11.96)and(48.77±13.95)pg/L of the control group,and the difference was statistically significant(P<0.05).The plasma Gas-17(90.08±22.52)ng/L and mucosal tissue Gas-17(303.20±50.94)ng/L of the experimental group were obviously lower than(99.79±24.56)and(329.57±55.12)ng/L of the control group,and the difference was statistically significant(P<0.05).Conclusion For patients with Hp infection and duodenal ulcer,combination of hydrotalcite and Hp can not only improve the active inflammation degree of antral mucosa,but also help to reduce the level of inflammatory mediators,and has a good regulatory effect on the level of gastrointestinal hormones.
