自拟温肾运脾汤对脾肾阳虚-寒湿瘀阻型痛风患者的临床疗效观察

Clinical observation on the effect of Self-designed Wenshen Yunpi Decoction on spleen-kidney Yang deficiency and cold-dampness-stasis type gout

目的探讨自拟温肾运脾汤治疗脾肾阳虚-寒湿瘀阻型痛风患者的临床疗效。方法将符合纳入标准的86例患者,随机分为2组,即对照组和治疗组,每组各43例,对照组给予西医常规治疗,治疗组在对照组的基础上加用自拟温肾运脾汤联合治疗,疗程1个月。比较两组患者关节症状及体征评分,中医证候评分,血尿酸(uric acid,UA)、C反应蛋白(C-reactive protein,CRP)、血沉(erythrocyte sedimentation rate,ESR)水平以及复发率和总有效率的不同。结果治疗1个月后,治疗组患者关节症状及体征评分、血尿酸、C反应蛋白、血沉的表达水平以及复发率均明显低于对照组,且总有效率较对照组明显升高,差异均具有统计学意义(P<0.05)。结论自拟温肾运脾汤能改善脾肾阳虚-寒湿瘀阻型痛风患者的临床症状,与西药联合运用比单独运用西药治疗疗效更显著。

Objective To investigate the clinical efficacy of self-made Wenshen Yunpi decoction in gout patients with spleen and kidney yang deficiency-cold dampness stasis.Methods The 86 patients who met the inclusion criteria were randomly divided into control group and treatment group,with 43 cases in each group.The control group was given conventional western medicine treatment,and the treatment group was treated with self-made Wenshen Yunpi decoction in addition to the treatment for the control group;the course of treatment was 1 month for both groups.The scores of joint symptoms and signs,traditional Chinese medicine syndrome scores,levels of blood uric acid and C-reactive protein,erythrocyte sedimentation rate,recurrence rate,and overall response rate were compared between the two groups of patients.Results After 1 month of treatment,the treatment group had significantly lower scores of joint symptoms and signs,levels of blood uric acid and C-reactive protein,erythrocyte sedimentation rate,and recurrence rate and a significantly higher overall response rate compared with the control group(P<0.05).Conclusion The self-made Wenshen Yunpi decoction can improve the clinical symptoms of gout patients with spleen and kidney Yang deficiency-cold dampness stasis,and its combination with western medicine is more effective than western medicine alone.
