LncRNA MINCR表达对失代偿期肝硬化患者预后的评估价值

Prognostic Value of Expression of LncRNA MINCR in Patients with Decompensated Cirrhosis

【目的】探讨LncRNA MINCR表达对失代偿期肝硬化患者预后的评估价值。【方法】选取2017年1月至2018年1月本院住院的失代偿期肝硬化患者120例,依据随访生存状态分为生存组与死亡组。采用实时荧光定量聚合酶链反应(qRT-PCR)检测LncRNA MINCR的相对表达量。用Pearson法分析LncRNA MINCR和肝硬化患者临床特征的相关性。采用受试者工作特征曲线(ROC)分析各指标对失代偿性肝硬化预后的诊断性能。采用COX回归分析影响患者预后的独立危险因素。【结果】Pearson分析显示,LncRNA MINCR表达水平与总胆红素(TBil)、国际标准化比值(INR)、CTP评分、终末期肝病模型(MELD)评分呈正相关(P<0.05),与白蛋白(A1b)、肌酐(Cr)及Na^(+)相关性不显著(P>0.05)。ROC曲线分析表明,LncRNA MINCR预测肝硬化患者死亡的最佳临界点为2.32,灵敏度为96.71%,特异度为90.50%,ROC曲线下面积(AUC)为0.973。多因素COX回归分析表明,LncRNA MINCR是失代偿肝硬化患者死亡的独立危险因素(1.13;95%CI:1.07〜1.19)。【结论】LncRNA MINCR对判断失代偿期肝硬化患者病情及预后具有一定价值,检测方法简便易行,值得推广。

[Objective]To investigate the prognostic value of expression of LncRNA MINCR in patients with decompensated cirrhosis.【Methods】From January 2017 to January 2018,120 patients with decompensated cirrhosis were selected and divided into survival group and death group according to the follow-up survival status.The relative expression of LncRNA MINCR was detected by real-time fluorescence quantitative polymerase chain reaction(qRT-PCR).Pearson method was used to analyze the correlation between LncRNA MINCR and the clinical characteristics of patients with cirrhosis.ROC was used to analyze the diagnostic performance of each index for the prognosis of decompensated cirrhosivs.Cox regression analysis was used to ana-lyze the independent risk factors influencing the prognosis of patients.【Results】Pearson analysis showed that the expression level of LncRNA MINCR was positively correlated with TBIL,INR,CTP and MELD(P<0.05),but not with ALB,Cr and Na(P〉0.05).ROC curve analysis showed that the best critical point of LncRNA MINCR for predicting death in patients with cirrhosis was 2.32,the sensitivity was 96.71%,the specificity was 90.50%,and the area under AUC curve was 0.973.Multivariate Cox regression analysis showed that LncRNA MINCR was an independent risk factor for death in decompensated cirrhosis(HR:1.13;95%CI:1.07-1.19).【Conclusion】LncRNA MINCR has a certain value in judging the condition and prognosis of pa-tients with decompensated cirrhosis.The method is simple and easy to use,which is worth popularizing.
