中心静脉导管穿刺置管引流与小切口置管引流治疗乳腺脓肿的疗效比较

目的评估中心静脉导管穿刺置管引流术治疗乳腺脓肿的效果.方法选取2016年7月至2020年7月哺乳期乳腺脓肿患者153例,其中中心静脉导管穿刺置管引流治疗80例,小切口置管引流治疗73例,比较两组患者的疗效.结果导管置管组患者在切口长度、术后24 h疼痛评分、换药次数、引流口愈合时间及患者满意度优于小切口置管组(P<0.001).结论两组置管引流方式对乳腺脓肿的治疗效果类似,但中心静脉导管穿刺置管引流具有外观优、疼痛程度轻、换药次数少及引流口愈合时间缩短的优点,患者满意度高,建议使用中心静脉导管穿刺置管引流术替代小切口置管引流术治疗乳腺脓肿.

Objective To evaluate the application and clinical effect of central venous catheter puncture and drainage for breast abscess.Methods A total of 153 lactating breast abscess patients who were adnmitted to Hangzhou Women's Hospital from July 1,2016 to July 1,2020 were selected as the research objects.80 cases were treated with central venous catheter puncture and drainage while 73 cases were treated with minimally invasive incision drainage,and the catheterization characteristics of the two groups were compared.Results Patients in the central venous catheter puncture and drainage group were significantly better than the minimally invasive incision drainage group in terms of incision length,pain score in 24 hours after surgery,number of dressing changes,drainage port healing time and patient satsfaction(P<0.001).Conclusion The two groups of catheter drainage methods have similar therapeutic effects on breast abscesses,but central venous catheter puncture and drainage has the advantages of more beautiful appearance,safe and efective,less pain,fewer dressing changes,and significantly shorter drainage port healing time.It is recommended to treat breast abscess with central venous catheter puncture and drainage.
