摘要
基于"阳化气,阴成形"理论,认为小儿哮喘的病机为肺脾肾常不足,阴成形太过致宿痰伏肺,阳化气失司则卫外不固,风邪引触伏痰发为哮喘。基于阴阳理论辨识小儿哮喘的发生发展,重视调和阴阳,明确小儿哮喘本虚标实的特点,适时扶阳固本,同时重视病理产物,分阶段从风、瘀、饮、热、津辨证施治。发作期祛风解痉平喘,兼顾扶助正气,缓解期降气、化痰、祛瘀、补益气津并用,调理肺脾肾三脏,消除伏痰宿根,以达到机体阴平阳秘的状态。附案例1则,以资验证。
This paper holds that the pathogenesis of children with asthma is frequent insufficiency of lung, spleen and kidney, the excess of yin constituting form causing retained phlegm latent in the lung, the dysfunction of yang transforming into qi resulting in weakness of defensive qi and exterior aspect, and wind pathogen inducing latent phlegm to asthma attack based on "yang transforming into qi while yin constituting form" theory.Based on the theory of yin and yang to differentiate the occurrence and development of children with asthma, we should pay attention to regulating yin and yang, clarify the characteristics of deficiency in origin and excess in superficiality of children with asthma, timely strengthening yang and consolidating the origin, attach importance to pathological products concurrently, and treat asthma in stages according to syndrome differentiation of wind, blood stasis, fluid retention, heat and fluid.In the attack stage, it should expel wind, relieve convulsion and asthma, giving consideration to strengthen healthy qi.In the remission stage, it should descend qi, resolve phlegm, remove blood stasis, replenish qi and fluid simultaneously, regulate the lung, spleen and kidney, and eliminate the persistent root cause of latent phlegm, whereby achieving the state of equilibrium between yin and yang.One case is attached for verification.
