中药塌渍法联合内服独活寄生汤治疗膝骨性关节炎的疗效及对血清软骨代谢标志物水平的影响

Effect of the traditional Chinese medicine dip-soaking therapy combined with Duhuo Jisheng Decoction on knee osteoarthritis and its influence on the level of the serum cartilage metabolism markers

目的观察中药塌渍法联合内服独活寄生汤治疗膝骨性关节炎的疗效及对血清软骨代谢标志物水平的影响。方法将2017年12月-2019年1月收治的92例膝骨性关节炎患者随机分为2组,对照组46例给予双氯芬酸联合中药塌渍法治疗;观察组46例在对照组治疗基础上给予独活寄生汤内服治疗,连续治疗4周。结果2组治疗后疼痛、僵硬及关节功能积分,験上囊积液和滑膜厚度,血清COMP、CTX-Ⅱ、MMP-3均明显降低(P<0.05),且观察组均明显低于对照组(PV0.05);观察组总有效率为93.5%(43/46),明显高于对照组的73.9%(34/46)(P<0.05);2组患者治疗期间均未出现明显不良反应。结论中药塌渍法联合内服独活寄生汤能够抑制膝骨性关节炎患者血清COMP、CTX-Ⅱ、MMP-3水平,改善关节病理性损伤,有助于缓解退行性关节症状,提高关节功能。

Objective To observe the curative effect of the traditional Chinese medicine dip-soaking therapy combined with the oral administration of Duhuo Jisheng Decoction in the treatment of knee osteoarthritis and its effect on the level of serum cartilage metabolism markers.Methods A total of ninety-two patients with knee osteoarthritis admitted in our hospital from December 2017 to January 2019 were randomly divided into 2 groups.The 46 patients in the control group were treated with diclofenac combined with the traditional Chinese medicine dip-soaking therapy,while the 46 patients in the observation group were treated with Duhuo Jisheng Decoction on the basis of the control group.The treatment of both groups continued for 4 weeks.Results After treatment,the scores of pain,stiffness and joint function,suprapatellar bursa effusion and synovial thickness,serums of COMP,CTX-Ⅱ and MMP-3 in the two groups were significantly decreased(P<0.05),and the observation group was significantly lower than the control group(P<0.05).The total effective rate of the observation group was 93.5%(43/46),which was significantly higher than the control group's 73.9%(34/46)(P<0.05).There were no significant adverse reactions in the two groups during the treatment.Conclusion The combination of the traditional Chinese medicine dip-soaking therapy with the oral administration of Duhuo Jisheng Decoction can inhibit the level of the serums COMP,CTX-Ⅱ and MMP-3 in the patients with knee osteoarthritis,improve the joint pathological injuries,help relieve the degenerative joint symptoms and improve the joint functions.
