桃红四物汤联合手法复位夹板外固定对Colles骨折患者血清ALP、BMP-2、血Ca水平及腕关节功能的影响

Effect of Taohong Siwu Tang Combined with Manual Reduction and Splint External Fixation on the levels of serum ALP,BMP-2,blood Ca and Wrist Joint Function in Patients with Colles Fracture

目的探讨桃红四物汤联合手法复位夹板外固定对Colles骨折患者血清碱性磷酸酶(ALP)、骨形态发生蛋白-2(BMP-2)、血钙(Ca)水平及腕关节功能的影响。方法2017年3月-2019年3月四川省骨科医院收治的Colles骨折,共120例,以随机数字表法分为对照组和研究组,各60例。对照组采用手法复位夹板外固定进行治疗,研究组在此基础上加用桃红四物汤治疗,治疗8周。比较2组叩击痛消失时间、骨折愈合时间和肿胀消褪时间,术前、术后8周的血清ALP、BMP-2、血Ca水平、腕关节功能、视觉模拟评分(VAS)评分及治疗期间的并发症发生率。结果研究组叩击痛消失、骨折愈合及肿胀消褪时间均短于对照组(P<0.05)。与术前比,术后8周,2组血清ALP、BMP-2、血Ca水平、腕关节功能评分均升高,研究组高于对照组(P<0.05);而VAS评分均降低,研究组低于对照组(P<0.05)。研究组并发症发生率为5.00%,低于对照组的20.00%(P<0.05)。结论桃红四物汤联合手法复位夹板外固定治疗Colles骨折可显著升高患者血清ALP、BMP-2、血Ca水平,改善腕关节功能,并能缓解患者疼痛,降低其并发症发生率,疗效显著。

Objective To explore the effect of Taohong siwu tang combined with manual reduction and splint external fixation on the levels of serum alkaline phosphatase(ALP),bone morphogenetic protein-2(BMP-2),blood calcium(Ca)and wrist joint function in patients with Colles fractures.Methods A total of 120 patients with Colles fractures admitted to the department of orthopedics in Sichuan orthopedic hospital between March 2017 and March 2019 were selected and divided into the control group and the research group according to the random number table method,with 60 cases in each group.The control group was treated with manual reduction splint external fixation,and the research group was additionally treated with Taohong siwu tang,treatment for 8 weeks.The time of disappearance of percussion pain,fracture healing and swelling fading;the levels of serum ALP,BMP-2,blood Ca,the scores of wrist function and visual analogue score(VAS)before and 8 weeks after surgery and the total complication rate during the two groups of treatment were compared.Results The time for percussion pain disappearance,fracture healing,and swelling fading were shorter in the research group than in the control group(P<0.05).Compared with preoperative,8 weeks after surgery,the levels of serum ALP,BMP-2,blood Ca and the score of wrist joint function in the two groups increased,the research group was higher than the control group(P<0.05),while the score of VAS in both groups decreased,the research group was lower than the control group(P<0.05).The complication rate in the research group was 5.00%,lower than 20.00%in the control group(P<0.05).Conclusion Taohong siwu decoction combined with manual reduction and splint external fixation for the treatment of distal radius fractures can obviously increase the levels of serum ALP,BMP-2,blood Ca of patients,improve wrist joint function,and can alleviate patients’pain,reduce the complication rate,and have a significant effect.
