摘要
目的探讨听觉诱发电位在胆红素脑病(ABE)早期诊断中的应用价值及检出率。方法选择2019年1月—2020年1月本院收治的ABE患者30例作为观察组,收集同期本院体检结果为健康者30名作为对照组,两组均实施听觉诱发电位检测,对比检测指标的变化情况。结果观察组异常率(83.33%)显著高于对照组(10.00%),对异常情况进行复筛发现观察组复通率(0.00%)显著低于对照组(100.00%)(P<0.05)。观察组PL指标与对照组Ⅰ~Ⅱ、Ⅲ、Ⅴ波潜伏期指标相比均高于对照组,观察组IPL指标在Ⅰ~Ⅲ、Ⅲ~Ⅴ、Ⅰ~Ⅴ波峰时间平均数值均高于对照组(P<0.05)。胆红素指标≥343μmol/L检测异常率为60.00%,257~342μmol/L之间异常率为83.33,≥343μmol/L检测异常率为100.00%,随着胆红素指标的升高检测异常率随之升高。结论胆红素脑病(ABE)早期诊断中使用脑干听觉诱发电位测试可以及时发现异常的情况,可以反应出耳蜗至脑干的功能以及结构,体现出听觉诱发电位的变化模式,可以及早发现损伤,及时进行疾病干预,降低胆红素脑病对患儿的影响。
Objective To investigate the application value and detection rate of auditory evoked potentials in the early diagnosis of bilirubin encephalopathy(ABE).Methods Thirty patients with ABE those admitted to our hospital from January 2019 to January 2020 were selected as the observation group,and 30 healthy peoplereceived physical examination in our hospital during the same period were collected as the control group.Evoked potential detection was carried out in both groups.The changes of detected indicators were compared.Results The abnormality rate was 83.33%in the observation group which was significantly higher than 10.00%in the control group.The re-screening of abnormal conditions found that the re-cancellation rate in the observation group was 0.00%,it was significantly lower than 100.00%of the control group(P<0.05).Compared with the control group,the PL index of the observation group was higher than the control group'sⅠ~Ⅱ,Ⅲ,Ⅴwave latency index,and the average value of the IPL index of the observation group was higher than the control group at the peak time ofⅠ~Ⅲ,Ⅲ~Ⅴ,andⅠ~Ⅴ(P<0.05).The abnormal detection rate of bilirubin index equal to or over343μmol/L was 60.00%,the abnormal rate between 257 and 342μmol/L was 83.33%,and the abnormal detection rate equal to or over343 umol/L was 100.00%.As the bilirubin index increased,the abnormal detection rate increases accordingly.Conclusions The use of brainstem auditory evoked potential test in the early diagnosis of bilirubin encephalopathy could detect abnormalities in time,reflect the function and structure of the cochlea to the brainstem,reflect the change pattern of auditory evoked potentials,and detect damage early,and promote timely intervention of the disease to reduce the impact of bilirubin encephalopathy on children.
