miRNA-137、KISS-1表达与结直肠癌根治术后复发转移的相关性研究

摘要 目的探讨结直肠癌患者行根治性手术后复发转移与微小RNA-137(miRNA-137)、肿瘤转移抑制基因KISS-1表达水平的关系。方法选择2013年1月-2015年5月在本院行根治性手术的103例Ⅱ期CRC患者癌组织和癌旁正常组织为研究对象。采用实时荧光定量法检测miRNA-137、KISS-1 mRNA表达水平;采用免疫组化法分析CRC患者KISS-1蛋白表达情况;分析CRC患者癌组织miRNA-137、KISS-1表达水平与临床病理特征的关系;分析CRC患者术后复发转移与癌组织miRNA-137、KISS-1蛋白水平的关系;Kaplan-Meier生存曲线分析CRC患者癌组织miRNA-137、KISS-1蛋白水平与预后的关系。结果与癌旁正常组织相比,CRC患者癌组织miRNA-137、KISS-1 mRNA表达水平、KISS-1蛋白阳性表达率均明显降低(P<0.05);miRNA-137、KISS-1蛋白水平与CRC患者肿瘤分化程度、T分期相关(P<0.05);CRC患者癌组织miRNA-137表达水平与KISS-1 mRNA表达水平呈正相关(P<0.05);CRC患者术后复发转移与癌组织miRNA-137表达水平、KISS-1蛋白水平显著相关(P<0.05);CRC患者miRNA-137高水平组、KISS-1阳性组术后60个月总生存率、无病生存率均明显高于miRNA-137低水平组、KISS-1阴性组(P<0.05)。结论CRC患者癌组织miRNA-137、KISS-1表达均明显下调,两者与CRC患者术后复发转移、预后均显著相关,检测结直肠癌组织miRNA-137、KISS-1水平有利于临床判定CRC患者预后。 Objective To investigate the relationship between the recurrence and metastasis after radical operation and expression of miRNA-137 and tumor metastasis suppressor KiSS-1 in patients with colorectal cancer(CRC).Methods From January 2013 to May 2015,the cancer tissue and adjacent normal tissue from 103 patients with stage II CRC who underwent radical surgery were selected as research subjects.Real time fluorescent quantitative PCR(QRT PCR)was used to detect miRNA-137 and KiSS-1 mRNA levels.The expression of miRNA-137 and KiSS-1 protein in CRC tissues was analyzed by immunohistochemistry;the relationship between the expression of miRNA-137 and KiSS-1 and clinicopathological features was analyzed;the relationship between the recurrence and metastasis of CRC patients and the protein levels of miRNA-137 and KiSS-1 was analyzed;the relationship between the protein levels of miRNA-137 and KiSS-1 and prognosis was analyzed by Kaplan-Meier survival curve.Results Compared with adjacent normal tissues,the expression levels of miRNA-137 and KiSS-1 mRNA and the positive expression rate of KiSS-1 protein in CRC tissues were significantly decreased(P<0.05);the expression levels of miRNA-137 and KiSS-1 protein were correlated with tumor differentiation and T stage of CRC patients(P<0.05);the expression levels of miRNA-137 and KiSS-1 protein were correlated with tumor differentiation and T stage of CRC patients(P<0.05);there was a positive correlation between the expression of miRNA-137 and KiSS-1 mRNA in cancer tissues(P<0.05);the recurrence and metastasis of CRC patients were significantly correlated with the expression of miRNA-137 and the protein level of KiSS-1(P<0.05);the 60 months overall survival(OS)and disease-free survival(DFS)of CRC patients with high level of miRNA-137 and KiSS-1 positive group were significantly higher than those with low level of miRNA-137 and KiSS-1 negative group(P<0.05).Conclusions The expressions of miRNA-137 and KiSS-1 in CRC tissues are significantly down regulated,and they are significantly correlated with postoperative recurrence and metastasis and prognosis of CRC patients.Detection of miRNA-137 and KiSS-1 levels in colorectal cancer tissues is helpful to determine the prognosis of CRC patients.
