摘要
目的探讨临床药师主导下的品管圈(QCC)活动对规范质子泵抑制剂(PPI)预防性使用的效果。方法成立QCC,按QCC活动原则对实施前(2018年10月至12月)预防性应用PPI的不合理使用原因进行分析并干预,计算并比较QCC活动实施前、实施后(2019年3月至7月)PPI预防性使用的合理率。结果住院患者预防性使用PPI的合理率由QCC实施前的7.83%升至实施后的69.39%,两者差异显著(P<0.01);普外科、神经外科预防性使用PPI的人均费用分别由实施前的735.1元和889.8元,减至实施后的190.4元和59.1元。临床药师在医院合理用药监管中的重要作用得到体现。结论临床药师主导下的QCC活动,不仅可显著提高PPI预防性使用的合理率,还能增强团队中医师和药师的协作精神,以及职能部门对临床药师工作的认可与支持。
Objective To investigate the role of quality control circle(QCC)activities led by clinical pharmacists in regulating prophylactic use of proton pump inhibitors(PPIs).Methods QCC was established,the causes of irrational prophylactic use of PPI before the implementation of the activity(from October to December 2018)were analyzed and intervened according to the principles of QCC activities.The rational prophylactic use rate of PPI was calculated and compared before and after the implementation(from March to July2019).Results Through QCC activities,the rational prophylactic use rate of PPI increased from 7.83%to 69.39%among hospitalized patients,there a was significant difference between before and after the QCC activities(P<0.01).The per capita cost of prophylactic use of PPI decreased from CNY 735.1 and CNY 889.8 in the general surgery department and CNY 190.4 and CNY 59.1 in the neuro surgery de partment.Clinical pharmacists played an important role in the supervision of rational drug use in the hospital.Conclusion QCC activities leading by clinical pharmacists can not only improve the rational prophylactic use of PPI significantly,but also improve the teamwork spirit of doctors and pharmacists in the team and enhance the recognition and support of functional departments for the work of clinical pharmacists.
