Establishment and Application of Integrated Pharmaceutical Care Model in the Construction of a Medical Consortium

目的探讨提高医联体建设中一体化药学服务模式质量的方法。方法采用文献研究、问卷调查、实地调研的方法,了解基层医疗机构药师服务的现状,制订一体化药学服务模式建设内容,分析医联体建设中一体化药学服务模式实施中存在的问题。结果建立"以三级医疗机构药师为主导,基层医疗机构药师为主体,患者为中心,服务群众健康为目的"的上下联动的一体化药学服务模式。结论一体化药学服务模式的建立,将三级医疗机构优质的药学资源下沉,提高了基层医疗机构药师的服务能力和服务水平,使基层医疗机构患者享有同质化、均值化的医疗服务,慢性疾病患者得到了居家药学服务,充分发挥了医联体建设的作用,促进了分级诊疗制度的实施。

Objective To investigate the methods to improve the quality of the integrated pharmaceutical care model in the construction of a medical consortium.Methods Literature research,questionnaire survey,and field investigation were conducted to understand the current status of pharmacist’s services in primary medical institutions,and analyze the problems existing in the implementation of the integrated pharmaceutical care mode in the construction of a medical consortium.Results An integrated pharmaceutical care model was established with″pharmacists in tertiary medical institutions as the main lead,and those in primary medical institutions as the main body,the patients as the center,and the purpose of serving public health″,joining the higher-level and lower-level medical institutions.Conclusion The establishment of an integrated pharmaceutical care model will distribute the high-quality resources of tertiary medical institutions over lower-level institutions,improve the service capabilities and service levels of pharmacists in primary medical institutions,and enable primary-level patients to enjoy equalized medical services and patients with chronic diseases to receive in-home pharmaceutical care,playing a big role in the construction of a medical consortium and promoting the implementation of hierarchical diagnosis and treatment.
