Exploration of the Innovative Pharmaceutical Care in the Wards by Using the Quality Management Tools

目的探讨药品质量管理工具用于医院病区药学服务的路径与方法。方法采用"鱼骨图"法分析影响病区药品质量安全的原因,将PDCA循环管理模式用于病区基数药品的检查,并开展契合临床需求的药学服务。结果病区药品的基数设置、标签规范化管理、科学储存管理、有效期统筹管理均得以优化;药师开展了对临床、对病区的"点对点""一对一"等临床安全用药指引工作。结论病区药品质量管理及药学服务工作的开展使病区药品管理更加规范化和同质化,多元化的药学服务使药品的临床使用及管理更加科学与合理,可确保患者的用药安全。

Objective To explore the application of path and method of medicine quality management tools for pharmaceutical care in the hospital wards.Methods The″fishbone diagram″method was used to analyze the reasons affecting the quality and safety of medicines in the wards,PDCA cycle management mode was used to check the base medicines in the wards,and pharmaceutical care according with the clinical needs was carried out.Results The setting of base medicines,standardized label management,scientific storage management and overall management of validity period of medicines in the wards were optimized.Pharmacists have carried out″point-to-point″and″one-to-one″clinical safety medication guidelines for the clinic and wards.Conclusion Carrying out drug quality management and pharmaceutical care in the wards make the ward medicine management more standardized and homogenous.The diversified pharmaceutical care makes the clinical use and management of medicines more scientific and reasonable,which can ensure the safety of patients’medication.
