CT引导下弹簧圈定位SPN行VATS的临床应用

摘要 目的:探究计算机断层扫描(Computerized tomogrphy,CT)引导下弹簧圈定位引导电视胸腔镜(videoassisted thoracoscopic surgery, VATS)手术精确切除孤立性结节(Solitary pulmonary nodule, SPN)的临床价值。方法:收集2017年5月至2019年5月周口市中心医院行VATS治疗的SPN患者32例,所有患者均于VATS术前行CT引导下经皮穿刺,于SPN病灶处放置弹簧圈定位,随后行VATs。记录弹簧圈定位成功率、CT引导放置弹簧圈时间、手术并发症和术后临床病理。结果:32例患者均定位成功,弹簧圈定位成功率100%,操作时间18.5 min;VATS手术成功率100%,术后病理结果12例为肺腺癌,4例为非典型样增生,10例为原位腺癌,8例炎性病变;手术并发症28.1%,其中出现2例出现少量渗血、3例无症状气胸、4例出现胸痛,均无需特殊处理。结论:SPN在术前采用CT引导放置弹簧圈定位成功率较高,无严重并发症,且有效促进VATS手术效果,有效提高SPN的临床治疗。
