2型糖尿病周围神经病变患者IGF-1水平变化及其临床意义 被引量:5

Clinical significance of IGF-1 levels in type 2 diabetic patients with peripheral neuropathy

摘要 目的探讨胰岛素样生长因子-1(IGF-1)在2型糖尿病周围神经病变(DPN)患者中的水平变化及临床意义。方法随机选取2020年2月—2020年7月在柳州市人民医院内分泌科住院的2型糖尿病(T2DM)患者94例,分为单纯T2DM患者36例(对照组),合并DPN患者58例(DPN组),分别检测两组的IGF-1、血清白蛋白(ALB)、空腹血糖(FPG)、胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、糖化血红蛋白(HbA1c)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、γ谷氨酰转肽酶(GGT)、同型半胱氨酸(HCY)、乳酸水平,进行组间t检验,采用pearson相关分析法比较IGF-1水平与DPN患者相关生化指标的相关程度。结果DPN组IGF-1、ALB、HDL水平明显低于对照组,差异有统计学意义(P<0.05),而TC、LDL、GGT、HCY、乳酸明显高于对照组,差异有统计学意义(P<0.05)。DPN组患者血IGF-1水平与Alb(r=0.371,P=0.049)呈正相关,且关系较为密切;与LDL-C(r=-0.215,P=0.021)、GGT(r=-0.136,P=0.036)呈负相关。结论DPN患者IGF-1水平降低,与白蛋白具有相关性,临床上可通过检测IGF-1与白蛋白水平变化辅助DPN的诊断及预测治疗效果。 Objective To explore the clinical significance of insulin-like growth factor-1(IGF-1) in type 2 diabetic patients with peripheral neuropathy(DPN).Methods A group of 94 type 2 diabetic patients hospitalized in the Endocrinology Department of Liuzhou People’s Hospital from February 2020 to July 2020 were divided into two groups: control group with only type 2 diabetes(n=36) and DPN group with type 3 diabetes combined with DPN(n=58). The IGF-1, serum albumin(ALB), fasting blood glucose(FPG), cholesterol(TC), triglycerides(TG), low-density lipoprotein(LDL-C), high-density lipoprotein(HDL-C), glycosylated hemoglobin(HbA1c), alanine aminotransferase(ALT), aspartate aminotransferase(AST), gamma glutamyl transpeptidase(GGT), homocysteine(HCY), lactic acid level were detected respectively in the two groups, followed with the test between the two groups and Pearson correlation analysis was used to study the correlation between IGF-1 level and relevant biochemical indicators of DPN patients.Results The levels of IGF-1, serum albumin, and HDL in the DPN group were significantly lower than that of the control group(P<0.05). The total cholesterol, LDL, GGT, HCY, and lactic acid were significantly higher than that of the control group(P<0.05). The blood IGF-1 level of the patients in the DPN group was positively correlated with Alb(r=0.371, P=0.049) and negatively with LDL-C(r=-0.215, P=0.021), GGT(r=-0.136, P=0.036).Conclusion The decrease of IGF-1 level in patients with DPN is related to albumin. Clinically, the detection of IGF-1 and albumin levels can assist the diagnosis of DPN and predict the treatment outcomes.
