右美托咪定在甲状腺腺瘤患者行甲状腺次全切除术术后镇静中的应用效果

Application effects of Dexmedetomidine in sedative anesthesia in patients with thyroid adenoma undergoing subtotal thyroidectomy

目的:观察右美托咪定在甲状腺腺瘤患者行甲状腺次全切除术术后镇静中的应用效果。方法:选取80例行甲状腺次全切除术的甲状腺腺瘤患者作为研究对象,按随机数字表法分为观察组和对照组各40例。两组均采用静脉全身麻醉,术后对照组采用丙泊酚镇静,观察组采用右美托咪定镇静,比较两组术后麻醉苏醒相关指标水平、苏醒时Ramsay镇静评分和躁动评分(RS),以及不良反应发生率。结果:观察组呼之睁眼时间、气管导管拔除时间和复苏室停留时间均明显短于对照组,差异有统计学意义(P<0.05);观察组Ramsay镇静评分高于对照组,RS评分低于对照组,差异均有统计学意义(P<0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:右美托咪定用于甲状腺腺瘤患者行次全切除术术后镇静中可提高Ramsay镇静评分,缩短术后麻醉苏醒相关指标水平,降低RS评分和不良反应发生率,其效果优于丙泊酚镇静麻醉。

Objective:To explore application effects of Dexmedetomidine in sedative anesthesia in patients with thyroid adenoma undergoing subtotal thyroidectomy.Methods:80 patients with thyroid adenoma,who underwent subtotal thyroidectomy,were selected as the research objects,and were divided into observation group and control group according to the random number table method,40 cases in each group.The control group was given Propofol for sedation,while the observation group received Dexmedetomidine.The relative index levels of postoperative anesthesia recovery time,Ramsay sedation and restlessness(RS)score during the recovery,and the incidence of adverse reactions were compared between the two groups.Results:The eye opening time,extubation time,and resuscitation room stay time of the observation group were significantly shorter than those of the control group,and the differences were statistically significant(P<0.05).The Ramsay sedation score of the observation group was higher than that of the control group,the RS score of the observation group was lower than that of the control group,and the differences were statistically significant(P<0.05).Further,the incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:Dexmedetomidine applied to the patients with thyroid adenoma undergoing subtotal resection can improve the Ramsay sedation score,shorten the relative index levels of postoperative anesthesia recovery,and reduce the RS score and the incidence of adverse reactions.Moreover,it is superior to Propofol sedative anesthesia.
