CT与MRI在早期强直性脊柱炎骶髂关节病变患者中的应用效果比较

Comparison of effects of CT and MRI in diagnosis of early ankylosing spondylitis sacroiliac joint lesions

目的:比较CT与磁共振成像(MRI)在早期强直性脊柱炎骶髂关节病变患者中的应用效果。方法:选取120例早期强直性脊柱炎骶髂关节病变患者作为研究对象,均行CT与MRI检查,比较CT与MRI对早期强直性脊柱炎骶髂关节病变症状的检出率和不同等级检出率。结果:MRI对骨髓水肿、关节面下骨质囊变、腰5骶1关节软骨肿胀检出率均高于CT,差异有统计学意义(P<0.05);两种检查方法的关节侵蚀、关节面增生硬化检出率比较,差异均无统计学意义(P>0.05);CT对0级强直性脊柱炎骶髂关节病变检出率高于MRI,Ⅰ级检出率低于MRI,差异均有统计学意义(P<0.05);两种检查方法对Ⅱ、Ⅲ、Ⅳ级强直性脊柱炎骶髂关节病变检出率比较,差异均无统计学意义(P>0.05)。结论:MRI对早期强直性脊柱炎骶髂关节病变症状的检出率和不同分级检出率均优于CT。

Objective:To compare effects of CT and MRI in diagnosis of early ankylosing spondylitis sacroiliac joint lesions.Methods:120 patients with early ankylosing spondylitis sacroiliac joint lesions were selected as the research objects,and all underwent CT and MRI.The detection rates of CT and MRI in early ankylosing spondylitis and sacroiliac joint lesion symptoms and the detection rates of different disease grades were compared.Results:The detection rates of MRI for bone marrow edema,subarticular bone cyst degeneration,and lumbosacral articular cartilage swelling were higher than those of CT,and the differences were statistically significant(P<0.05).There were no statistically significant differences between the two methods in the detection rate of joint erosion and articular surface hyperplasia and sclerosis(P>0.05).The detection rate of CT for the grade 0 ankylosing spondylitis sacroiliac joint lesions was higher than that of MRI;the detection rate of grade I was lower than that of MRI;and the differences were statistically significant(P<0.05).However,there were no statistically significant differences in the detection rate of ankylosing spondylitis sacroiliac joint lesions of grade II,III,and IV between the two methods(P>0.05).Conclusions:MRI is superior to CT in the detection rate of early ankylosing spondylitis sacroiliac joint lesion symptoms and the detection rate of different disease grades.
