无肌松联合喉罩对小儿眼科短小手术后苏醒期的影响

Effect of non-muscle relaxation combined with laryngeal mask on recovery period in children undergoing short-duration ophthalmic surgery

目的探讨无肌松联合喉罩在小儿眼科短小手术中的应用效果,观察其对苏醒期的影响。方法选取我院2020-06/2020-09期间住院行眼科斜视矫正手术的患儿80例为研究对象,采用随机数字表法均衡分为对照组(A组,n=40)和试验组(B组,n=40)。A组使用肌松药常规静脉诱导,B组不使用肌松药诱导,2组均使用喉罩通气。对比2组患者各项指标变化情况及安全性。结果与A组对比,B组患儿平均动脉压(MAP)指标在观察术毕(T_(1))、拔管时(T_(2))、拔管后10 min(T_(3))时间点差异有统计学意义(P<0.05),心率(HR)、呼吸频率(RR)在T_(1)、 T_(2)时间点差异均有统计学意义。两组患儿拔管时间及定向力恢复时间比较有统计学意义(P<0.05)。A组患儿不良反应发生率高于B组,组间比较差异有统计学意义(P<0.05)。结论在小儿短小眼科手术中,无肌松药联合喉罩可安全、有效应用,同时可使患儿苏醒更快,拔管时间更短,有效降低苏醒期的喉痉挛等不良反应发生率。

Objective To investigate the application effect of non-muscle relaxant combined with laryngeal mask airway(LMA) in pediatric ophthalmic surgery, and to observe its effect on patients during recovery period. Methods Altogether 80 children with strabismus in our hospital from June 2020 to September 2020 were randomly divided into the control group(group A, n=40) and the experimental group(group B, n=40). In group A, muscle relaxants were used for routine intravenous induction, while in group B, muscle relaxants were not used for induction. The changes of each index and safety of the two groups were compared. Results Compared with group A, there were significant differences in MAP and HR at T1, T2 and T3 in group B(P<0.05), and there were significant differences in RR indexes at T1 and T2(P<0.05). There were significant differences in extubation time and orientation recovery time between the two groups(P<0.05). The incidence of adverse reactions in group A was significantly higher than that in group B(P<0.05). Conclusion Non-muscle relaxant combined with LMA can be used safely and effectively in children’s short-duration ophthalmic surgery. At the same time, it can make children wake up faster, shorten extubation time and effectively reduce the incidence of laryngeal spasm and other adverse reactions during recovery period.
