272例新型冠状病毒肺炎患者胸部CT演变特点分析

Retrospective Analysis of Chest CT of 272 Cases with Coronavirus Disease 2019(COVID-19)

目的:探讨新型冠状病毒肺炎的胸部CT特点,以评价CT对新型冠状病毒肺炎的临床分型及病情评估的价值。方法:收集广州市第八人民医院2020年1—3月共272例新型冠状病毒2019-nCoV核酸检测阳性患者的临床数据及胸部CT数据,进行回顾性分析。结果:新型冠状病毒肺炎重症患者与非重症患者的胸部CT影像学表现在病变部位、病变类型、发展趋势均有所差别,有统计学意义(P<0.05)。重症组病例肺部病变均累及双肺,非重症组病例可出现仅累及单侧肺病变,胸腔积液在重症病例上更为多见,重症组肺部病变胸部CT显示病变渗出时间较非重症组时间长。结论:胸部CT的影像学证据对疾病诊断起着重要的作用,在辅助临床进行疾病分型和评估治疗效果等方面作用明显,在疾病发展前期评估病情变化方面,要较外周血实验室辅助检查更具特异性,对临床采取干预治疗的时机提供了重要参考。

Objective:To investigate chest CT performance,assess the value of chest CT for evaluating the clinical classification and identifying the condition of patients with COVID-19.Methods:Retrospective analysis of the clinical and chest CT data of 2722019-nCoV positive patients,collected from January 2020 to March 2020 in Guangzhou Eighth People’s Hospital.Results:There were significant differences in the lesion location,types and development trend of chest CT findings between common patients and severe patients with COVID-19(P<0.05).In the severe group,the lung lesions involved both lungs.In the common patients,only one lung lesion was involved.Pleural effusion was more common in the severe patients.The exudation time of lung lesions in the severe patients was longer than that in the common patients.Conclusion:Chest CT plays an important role in the diagnosis of COVID-19,which contributes to definitely type the disease and evaluating the treatment effect.It can provide an early warning of the deteriorative disease and an important reference for the timing of clinical intervention.
