关节镜下膝关节清理术结合去神经化治疗对膝骨关节炎合并软骨损伤患者血清软骨代谢产物及相关基质金属蛋白酶水平的影响 被引量:3

摘要 目的分析关节镜下膝关节清理术结合去神经化治疗对膝骨关节炎合并软骨损伤患者血清软骨代谢产物及相关基质金属蛋白酶水平的影响。方法将我院收治的200例膝骨关节炎合并软骨损伤患者根据不同治疗方案分为对照组(关节镜下膝关节清理术)和试验组(在对照组基础上结合去神经化治疗),各100例。比较两组的血清软骨代谢产物、相关基质金属蛋白酶水平、疼痛情况、并发症发生情况及膝关节功能。结果术后,试验组血清CTX-Ⅱ、COMP、MMP-13、MMP-9水平均低于对照组(P<0.05)。试验组术后12、24 h的VAS评分及并发症总发生率均低于对照组(P<0.05)。试验组HSS各项评分均高于对照组(P<0.05)。结论关节镜下膝关节清理术结合去神经化治疗能有效降低膝骨关节炎合并软骨损伤患者的血清CTX-Ⅱ、COMP、MMP-13、MMP-9水平,减轻术后疼痛感,降低术后并发症发生率,提高膝关节功能。 Objective To analyze the effects of arthroscopic debridement of the knee joint combined with denervation treatment on levels of serum cartilage metabolites and related matrix metalloproteinases in patients with knee osteoarthritis complicated with cartilage injury.Methods A total of 200 knee osteoarthritis with cartilage injury patients admitted in our hospital were divided into control group(arthroscopic debridement of the knee joint)and experimental group(denervation treatment on the basis of the control group)according to different treatment schemes,with 100 cases in each group.The levels of serum cartilage metabolites and related matrix metalloproteinases,pain,complications and knee joint function were compared between the two groups.Results After operation,the levels of CTX-Ⅱ,COMP,MMP-13 and MMP-9 in the experimental group were lower than those in the control group(P<0.05).The VAS score at 12 and 24 h after operation and the total incidence of complications in the experimental group were lower than those in the control group(P<0.05).The HSS scores in the experimental group were higher than those in the control group(P<0.05).Conclusion Arthroscopic debridement of the knee joint combined with denervation treatment can effectively reduce the levels of serum CTX-Ⅱ,COMP,MMP-13 and MMP-9 in patients with knee osteoarthritis and cartilage injury,relieve postoperative pain,reduce the incidence of complications and improve knee joint function.
