综合护理干预在妊娠期糖尿病并发妊娠期高血压患者中的应用效果

Application effect of comprehensive nursing intervention in patients with gestational diabetes mellitus complicated with gestational hypertension

目的探讨综合护理干预在妊娠期糖尿病并发妊娠期高血压患者中的应用效果。方法选择2019年6月至2020年6月于本院门诊确诊的120例妊娠期糖尿病并发妊娠期高血压患者,使用Excel软件将其随机分为对照组与观察组,各60例。对照组给予常规护理,观察组在对照组的基础上给予综合护理干预。比较两组的血糖指标、血压、妊娠结局、新生儿并发症发生情况及护理依从性。结果观察组护理后的FBG、2 h PBG水平及HbA1c、收缩压及舒张压均低于对照组(P<0.05)。观察组的剖宫产、羊水过多及产后出血的发生率均低于对照组(P<0.05)。观察组的胎儿窘迫、新生儿低血糖、早产儿、新生儿窒息、新生儿黄疸的发生率低于对照组(P<0.05)。观察组的控制体重、适当运动、食盐控制及规律用药的护理依从性均优于对照组(P<0.05)。结论给予妊娠期糖尿病合并妊娠期高血压患者综合护理干预,可改善患者的血糖和血压控制情况,改善妊娠结局,降低新生儿并发症发生率,提高患者的护理依从性。

Objective To explore the application effect of comprehensive nursing intervention in patients with gestational diabetes mellitus complicated with gestational hypertension.Methods A total of 120 cases of gestational diabetes mellitus complicated with gestational hypertension diagnosed in our hospital from June 2019 to June 2020 were randomly divided into control group and observation group by using Excel software,with 60 cases in each group.The control group was given routine nursing,and the observation group was given comprehensive nursing intervention on the basis of the control group.The blood glucose indexes,blood pressure,pregnancy outcome,neonatal complications and nursing compliance were compared between the two groups.Results The levels of FBG,2 h PBG,HbA1c,systolic blood pressure and diastolic blood pressure in the observation group after nursing were lower than those in the control group(P<0.05).The incidences of cesarean section,polyhydramnios and postpartum hemorrhage in the observation group were lower than those in the control group(P<0.05).The incidences of fetal distress,neonatal hypoglycemia,premature infants,neonatal asphyxia and neonatal jaundice in the observation group were lower than those in the control group(P<0.05).The nursing compliance in terms of weight control,proper exercise,salt control and regular medication in the observation group were better than those in the control group(P<0.05).Conclusion Comprehensive nursing intervention for patients with gestational diabetes mellitus and gestational hypertension can improve the control of blood glucose and blood pressure,ameliorate the pregnancy outcome,reduce the incidences of neonatal complications,and enhance the nursing compliance of patients.
