分叉交锁髓内钉治疗肱骨骨折的效果及对患者血清BGP、CTX、OPG表达的影响

Effect of Bifurcation Interlocking Intramedullary Nail on Humeral Fracture and Expression of Serum BGP,CTX and OPG in Patients

目的:探讨分叉交锁髓内钉(BLIN)治疗肱骨骨折的效果及对患者血清BGP、CTX、OPG表达的影响。方法:选取2018年1月-2020年1月本院收治的94例肱骨骨折患者,按照随机数字表法分为观察组与对照组,每组47例。对照组给予锁定加压钢板(LCP)治疗,观察组给予分叉交锁髓内钉(BLIN)治疗。比较两组术中及术后情况、肩功能优良率、骨代谢活性指标及术后6个月并发症发生情况。结果:观察组术中出血量、切口长度均少于对照组,手术、住院、骨折线消失、骨骼愈合时间均短于对照组,差异均有统计学意义(P<0.05)。观察组肩功能优良率为91.49%,高于对照组的74.47%(P<0.05)。术前,两组血清BGP、CTX、OPG表达含量比较,差异均无统计学意义(P>0.05)。术后6个月,两组BGP、OPG、CTX表达含量均优于术前,且观察组BGP、OPG表达含量高于对照组,CTX表达含量低于对照组(P<0.05)。观察组术后6个月并发症发生率为4.26%,低于对照组的21.28%(P<0.05)。结论:分叉交锁髓内钉治疗肱骨骨折的效果较好,可减少手术创伤,降低并发症发生率,提升肩关节功能优良率,改善骨代谢活性。

Objective:To investigate the effect of bifurcation interlocking intramedullary nail(BLIN)on humeral fracture and the expression of serum BGP,CTX and OPG in patients.Method:A total of 94 patients with humeral fracture treated in our hospital from January 2018 to January 2020 were selected and divided into the observation group and the control group according to random number table method,47 cases in each group.The control group was treated with locking compression plate(LCP),and the observation group was treated with BLIN.The intraoperative and postoperative conditions,excellent and good shoulder function rate,bone metabolic activity index and complications within 6 months were compared between two groups.Result:The amount of intraoperative blood loss and incision length of the observation group were less than those of the control group,and the operation time,hospital stay,time of fracture line disappearance and time of bone healing of the observation group were shorter than those of the control group,with statistical significance(P<0.05).The excellent and good shoulder function rate of the observation group was 91.49%,which was higher than 74.47%of the control group(P<0.05).Before surgery,there were no significant differences in the expression levels of BGP,CTX and OPG in serum between two groups(P>0.05).6 months after surgery,the expression levels of BGP,CTX and OPG in two groups were superior than those of before surgery,and the expression levels of BGP and OPG of the observation group were higher than those of the control group,and the expression levels of CTX in the observation group were lower than those of the control group(P<0.05).6 months after surgery,the incidence of complications of the observation group was 4.26%,which was lower than 21.28%of the control group(P<0.05).Conclusion:Bifurcated interlocking intramedullary nail has a good effect on the treatment of humeral fracture,which can reduce surgical trauma,reduce the incidence of complications,improve the good shoulder function rate,and improve bone metabolic activity.
