食管癌术后早期肠内营养支持的临床效果

Clinical effects of early enteral nutrition support for patients after esophageal cancer surgery

目的探讨术后早期应用肠内营养支持治疗对食管癌患者预后的影响。方法选取2016年10月至2017年12月于我院行食管癌手术的患者40例,分为全肠外营养组(TPN组)与肠内营养组(EN组),每组20例,TPN组患者术后7 d内给予全肠外营养支持治疗,EN组患者手术24 h后给予肠内营养支持治疗。比较2组患者术后2 d、5 d和8 d血清白蛋白和白细胞计数的变化情况,并对术后胸部引管拔管时间、术后住院时间、肠道排气时间进行分析比较。结果TPN组术后5 d和8 d的血清白蛋白、白细胞计数均低于EN组,差异有统计学意义;EN组术后肠道排气时间、胸引管拔管时间及术后住院时间均少于TPN组,差异有统计学意义。结论食管癌术后早期给予肠内营养支持治疗,能够降低术后感染风险、促进肠道功能恢复、缩短胸引管拔管和住院时间,促进患者快速康复。

Objective We aimed to investigate the effect of early enteral nutrition support for patients after esophageal cancer surgery.Methods We included 40 patients who underwent esophageal cancer surgery and our selection criteria from October 2016 to December 2017.The patients were divided into the TPN group and the EN group,each containing 20 participants.We compared the changes in serum albumin levels and white blood cell counts on the 2nd,5th,and 8th postoperative days between the two groups.The intestinal venting time,time of chest tube drawing,and length of hospital stay were also compared between the two groups.Results The serum albumin levels and white blood cell counts on the 5th and 8th postoperative days in the TPN group were significantly lower than those in the EN group.The intestinal venting time,time of chest tube drawing,and length of hospital stay were all significantly lower in the EN group than in the TPN group.Conclusion Early enteral nutrition support for patients after esophageal cancer surgery can reduce the risk of postoperative infection,promote the recovery of intestinal function,shorten the retention time of the chest tube,shorten hospital stay,and promote the rapid recovery of these patients.
