摘要
目的探索鼻吸气压(SNIP)评价慢性阻塞性肺疾病(COPD)吸气肌力有效性以及SNIP与COPD患者病情严重程度的相关性。方法前瞻性、随机、对照入组设计,纳入49例COPD稳定期男性患者为病例组,20名健康男性为对照组。测定对照组及病例组的身体特征、肺功能、呼吸肌强度(SNIP、MIP)、6分钟步行距离(6MWD),对SNIP与肺功能、6MWD进行分析。结果病例组的SNIP值低于对照组(t=-2.733,P<0.05)。病例组SNIP与MIP、6MWD、第1秒用力呼气容量占预计值百分比(FEV1%pred)、第1秒用力呼气容积/用力肺活量(FEV1/FVC)各项指标均显著相关(r=0.837、0.509、0.512、0.563,P值均<0.05)。结论SNIP可有效的评价COPD患者吸气肌功能。COPD患者SNIP值较正常人群明显下降。SNIP与COPD严重程度存在关联,可以成为COPD病情综合评估的参考指标。
Objective Explored the effectiveness of sniff nasal inspiratory pressure(SNIP)in evaluating inspiratory muscle strength of chronic obstructive pulmonary disease(COPD)and the correlation between snip and the severity of COPD patients.Methods In a prospective,randomized,controlled design,49 male patients with stable COPD were enrolled as the case group and 20 healthy men as the control group.The physical characteristics,lung function,respiratory muscle strength(SNIP,MIP)and 6-minute walking test(6MWD)of the control group and the case group were measured.The correlation between SNIP and lung function,6MWD was analyzed statistically.Results The SNIP value of case group was lower than that of control group(t=-2.733,P<0.05).In case group,SNIP was significantly correlated with MIP,6MWD,forced expiratory volume in the first seccond in predicted(FEV1%pred)and FEV1/forced vital capacity(FVC)(r=0.837,0.509,0.512,0.563,all P<0.05).Conclusions SNIP can effectively evaluate the inspiratory muscle function of COPD patients effectively.The SNIP value of COPD patients was significantly lower than that of normal people.SNIP is associated with the severity of COPD and can be used as a reference index for comprehensive evaluation of COPD.
