2型糖尿病患者颈动脉病变与血脂、臂踝脉波传速、踝肱指数的关系

RELATIONSHIP BETWEEN CAROTID DISEASE AND BLOOD LIPIDS, BRACHIAL ANKLE PULSE WAVE VELOCITY, ANKLE BRACHIAL INDICES IN PATIENTS WITH TYPE 2 DIABETES

目的探讨2型糖尿病患者颈动脉病变与血脂、臂踝脉搏波传导速度、踝肱指数的关系。方法选择淮安市涟水县人民医院2型糖尿病患者183例,进行颈动脉超声检查、血样检测、动脉硬化检测,分析颈动脉病变与血脂、臂踝脉搏波传导速度、踝肱指数的相关性。结果颈动脉正常组的低密度脂蛋白水平显著低于颈动脉狭窄组(P<0.05),颈动脉增厚组的低密度脂蛋白胆固醇水平分别显著低于颈动脉斑块组(P<0.05)和颈动脉狭窄组(P<0.05);颈动脉狭窄组的载脂蛋白B水平显著高于颈动脉正常组(P<0.05)和颈动脉增厚组(P<0.05);踝肱指数异常者相对于正常者,颈动脉发生病变的风险增加(OR=3.74, 95%CI=1.37~10.23)。结论低密度脂蛋白胆固醇、载脂蛋白B、踝肱指数与2型糖尿病患者颈动脉病变有一定关系,其水平的改变可以提示2型糖尿病患者颈动脉粥样硬化的发生发展。[营养学报,2021,43(1):49-53]

Objective To explore the relationship between carotid disease and blood lipids, brachial ankle pulse wave velocity, ankle brachial indexes in patients with type 2 diabetes. Methods A total of 183 patients with type 2 diabetes were selected in People’s Hospital of Lianshui County, Huaian City. Carotid artery ultrasound check was performed while blood samples and arteriosclerosis were detected. The correlations between carotid artery disease and blood lipids, brachial ankle pulse wave velocity as well as ankle brachial indexes were analyzed. Results The level of low density lipoprotein cholesterol was significantly lower in the normal carotid group than in the stenosis carotid group(P<0.05), and in the thickened carotid group than in the plaque carotid group(P<0.05) and stenosis carotid group(P<0.05). The level of apolipoprotein B in the stenosis carotid group was significantly higher than in the normal carotid group(P<0.05) and thickened carotid group(P<0.05). Compared to the people with normal ankle brachial indexes, those with abnormal ones had an increased risk of carotid artery disease(OR=3.74, 95%CI=1.37~10.23). Conclusion Low density lipoprotein cholesterol, apolipoprotein B and ankle brachial indexes are related to carotid plaque formation in patients with type 2 diabetes. Changes in these parameters indicate the occurrence and development of carotid atherosclerosis in patients with type 2 diabetes. [ACTA NUTRIMENTA SINICA, 2021, 43(1):49-53]
