中医人文视域下关于医患分歧的思考

Thoughts on the doctor-patient divergences from the humanistic perspective of traditional Chinese medicine

叙事医学提出了4种医患分歧,分别是对死亡的认识、对病因的认识、疾病的情境及羞耻、责备、恐惧等情绪,但笔者发现在中医视域下,医患分歧受中国文化的影响有所不同。故本文从医患间具有中国传统文化的同源性、中医诊疗模式具有人文属性、中医学重视心身时空的整体性、传统文化视域下医患具有更加宏远的生死观等几个方面进行讨论,寻找具有中国独特医患分歧的原因,发现中医在弥合医患分歧中存在的隐性优势,为当今临床实践提供借鉴。

Four kinds of doctor-patient divergences have been put forward in narrative medicine, namely, recognition of death and of etiology, the context of disease and negative emotions such as shame, blame and fear. However, from the perspective of traditional Chinese medicine(TCM), the doctor-patient divergences, influenced by Chinese culture, shows its own characteristics. This study was aimed at investigating the influencing factors on the doctor-patient divergences with Chinese characteristics, in the following aspects: the homology of traditional Chinese culture between doctors and patients, humanity in diagnosis and treatment of TCM, psychosomatic and spatiotemporal characteristics of the TCM model, the extension of TCM view about life and death from the perspective of traditional Chinese culture, in order to shed light on the hidden advantages of TCM in bridging doctor-patient divergences, and providing reference for clinical practice.
