克拉霉素联合布地奈德对慢性鼻-鼻窦炎伴鼻息肉患者鼻内镜术后的康复作用

Effects of clarithromycin combined with budesonide on patients with chronic rhinosinusitis complicated with nasal polyps after nasal endoscopic surgery

目的:探究克拉霉素联合布地奈德喷鼻对慢性鼻-鼻窦炎伴鼻息肉患者鼻内镜术后的康复治疗作用。方法:选取南阳市中心医院收治的慢性鼻-鼻窦炎伴鼻息肉患者120例作为研究对象,按照随机数字表法分为对照组和观察组,每组60例。对照组给予布地奈德喷鼻治疗,观察组在对照组的基础上加用克拉霉素治疗。观察、分析治疗1个月后的临床效果、鼻气道阻力、黏膜功能和鼻黏膜相关因子。结果:治疗后,观察组总有效率98.3%,高于对照组85.0%,差异有统计学意义(P<0.05);鼻气道阻力值及黏膜功能评分低于对照组(P<0.05);VEGF、Eotaxin、MUC5AC表达低于对照组(P<0.05)。结论:克拉霉素联合布地奈德喷鼻可改善慢性鼻-鼻窦炎伴鼻息肉患者鼻内镜术后的黏膜功能,降低鼻气道阻力,提高治疗效果。

Objective:To investigate the effects of clarithromycin combined with budesonide nasal spray on the mucosa and ventilation function of patients with chronic rhinosinusitis with nasal polyps(CRSwNP)after nasal endoscopic surgery.Methods:120 CRSwNP patients admitted to our hospital were selected and randomized into two groups,with 60 cases in each group.The control group were given budesonide nasal spray for treatment,and on this basis,the observation group were treated with clarithromycin.The clinical efficacy,nasal airway resistance,mucosal function and nasal mucosal-related factors were compared between the two groups after 1 month of treatment.Results:After treatment,the total effective rate of the observation group was 98.3%,higher than 85%of the control group,which was of statistical significance(P<0.05);the nasal airway resistance and mucosal function scores were lower than those in the control group(P<0.05);the expressions of VEGF,Eotaxin and MUC5 AC in the observation group was lower than those of the control group(P<0.05).Conclusion:Clarithromycin combined with budesonide nasal spray can improve the mucosal function in chronic rhinosinusitis patients complicated with nasal polyps after endoscopic nasal surgery,reduce nasal airway resistance and improve the therapeutic effect.
