同一体检人群2年脂肪肝与代谢综合征相关指标关系的研究

A 2-Year Study on the Relationship between Fatty Liver and Metabolic Syndrome in the Same Physical Examination Population

目的:通过分析上海市金山区同一批体检人群两次健康体检结果,探讨非酒精性脂肪性肝病(NAFLD)的患病率、转归情况以及NAFLD与代谢综合征各组分指标间的关系。方法:对2017年、2019年连续参加上海市金山区居民体检的3 769例居民进行肝脏超声和人群体格、血生化检查。结果:2017年、2019年该人群NAFLD检出率分别为28.10%和18.17%。相对于两次均阴性组,两次均阳性组检出代谢综合征风险最高。随着Met S异常成分检出数量增加,研究对象患NAFLD的风险增加。多因素分析,超重或肥胖对NAFLD影响最大,其次为血脂紊乱、糖尿病和高血压,年龄是保护因素。结论:该地区NAFLD检出率与年龄相关,女性检出率高于男性。NAFLD与MetS各组分关系密切,体质指数对NAFLD影响最大,NAFLD防治应重点关注肥胖人群。

OBJECTIVE By analyzing the results of two physical examination of the same group of people in Jinshan District of Shanghai,it discussed the prevalence and prognosis of nonalcoholic fatty liver disease(NAFLD),as well as the relationship between NAFLD and each component index of metabolic syndrome. METHODS 3 769 residents who participated in the physical examination of Jinshan District residents in 2017 and 2019 were examined by liver ultrasound,physical examination and blood biochemical examination. RESULTS The detection rates of NAFLD in 2017 and 2019 were 28.10% and 18.17%,respectively. Compared with negative group,NAFLD positive group had the highest risk of METS detection. As the number of METS abnormal components increased,the risk of NAFLD increased. In the multi-factor analysis of NAFLD,overweight or obesity had the greatest impact on NAFLD. Dyslipidemia, diabetes and hypertension, and age were the protective factors. CONCLUSIONS The detection rate of NAFLD in the area was related to age,and the detection rate of female was higher than that of male. NAFLD was closely related to METS. BMI had the greatest impact on NAFLD,so the prevention and treatment of NAFLD should focus on obese people.
