心电图对左冠状动脉异常起源于肺动脉的早期诊断价值

摘要 目的探讨心电图对左冠状动脉异常起源于肺动脉(anomalous origin of the left coronary artery from the pulmonary artery,ALCAPA)的早期诊断价值。方法选取经手术证实的ALCAPA住院患者16例,以及15例经冠状动脉造影术诊断的冠状动脉肺动脉瘘(coronarypulmonary artery fistula,CAPAF)患者,比较上述两组的一般情况及心电图特征。结果两组间性别比较,差异无统计学意义;年龄及心电图特征比较,差异存在统计学意义。ALCAPA患者发病年龄早于CAPAF患者,前者心电图主要表现为Ⅰ、aVL导联异常Q波及ST-T改变,而后者心电图多无特征性表现。结论 ALCAPA患者发病年龄小、心电图特征明显,有助于疾病的早期诊断。 Objective To explore the early diagnostic value of ECG in anomalous origin of the left coronary artery from the pulmonary artery(ALCAPA).Methods We selected 16 inpatients diagnosed with ALCAPA by surgery and 15 patients diagnosed with coronary-pulmonary artery fistula(CAPAF)by coronary arteriography.They were divided into two groups;their general data and ECG characteristics were analyzed.Results There is no significant difference in gender between the two groups,but there are significant differences in age and ECG features.The onset age of ALCAPA patients is earlier than that of CAPAF patients;ECGs of the former are mainly manifested by abnormal Q waves in leadsⅠand aVL,and ST-T changes while ECGs of the latter seldom show characteristic manifestation.Conclusion ALCAPA patients are associated with younger onset age and obvious ECG characteristics which helps to make early diagnosis.
