胶质瘤组织UCH37表达水平与病人预后的相关性

摘要 目的探讨胶质瘤组织泛素化羧基末端水解酶37(UCH37)表达水平与病人预后的相关性。方法选取2017年6月至2019年6月手术切除的脑胶质瘤组织104例和瘤旁脑组织64例,采用免疫组化法检测组织UCH37表达水平,根据免疫组化染色评分分成高表达组、低表达组;采用PCR法检测组织UCH37 mRNA水平。术后随访18个月,记录死亡、生存情况。结果104例中,死亡20例,存活84例;UCH37高表达62例,低表达42例。脑胶质瘤组织UCH37高表达率(59.62%)明显高于瘤旁组织(17.19%;P<0.05),而且脑胶质瘤组织UCH37 mRNA表达量明显高于瘤旁组织(P<0.05)。多因素logistic回归分析显示UCH37高表达是脑胶质瘤术后死亡的独立危险因素(P<0.05)。UCH37高表达组中位生存期(9个月)较低表达组(13个月)明显缩短(P<0.05)。ROC曲线分析显示,UCH37 mRNA评估病人术后死亡的最佳界值为2.725,曲线下面积为0.797(95%置信区间0.695~0.899),敏感度为70.00%,特异度为70.10%。结论脑胶质瘤UCH37呈高表达,表达水平越高,预后越差。肿瘤组织UCH37 mRNA水平检测对病人预后有一定评估价值。 Objective To explore the correlation between the expression level of ubiquitin C-terminal hydrolase 37(UCH37)in glioma tissues and the prognoses of glioma patients.Methods The expression levels of UCH37 protein and mRNA were detected in gliona tissues obtained from 104 glioma patients and in brain tissues adjacent to glioma of 64 glioma patients who underwent surgery from June 2017 to June 2019 were detected by immunohistochemical method and PCR,respectively.According to the immunohistochemical staining score,104 glioma patients were divided into high expression group(n=62)and low expression group(n=42).The postoperative follow-up was 18 months,and the death and survival were recorded.Results Of 104 patients,20 died and 84 survived.The high expression rate of UCH37 in glioma tissues(59.62%)was significantly higher than that(17.19%)in brain tissues adjacent to glioma(P<0.05),and the level of UCH37 mRNA in glioma tissues was significantly higher than that in brain tissues adjacent to glioma(P<0.05).Multivariate logistic regression analysis showed that the high expression of UCH37 was an independent risk factor for postoperative death of glioma patients(P<0.05).The median survival time(9 months)in the UCH37 high expression group was significantly shorter than that(13 months)in the lower expression group(P<0.05).ROC curve analysis showed that the best cut-off value of UCH37 mRNA for evaluation of postoperative death of glioma patients was 2.725,the area under the curve was 0.797(95%confidence interval 0.695~0.899),the sensitivity was 70.00%,and the specificity was 70.10%.Conclusions UCH37 is highly expressed in gliomas.The higher the expression level,the worse the prognosis.The detection of UCH37 mRNA level in tumor tissue has a certain evaluation value for the prognosis of glioma patients.
