补中益气汤加减联合生物反馈治疗气虚下陷型直肠黏膜内脱垂25例

Modified Buzhong Yiqi Tang Combined with Biofeedback in the Treatment of Internal Rectal Prolapse with Qi Deficiency and Skinking:Effect Observation on 25 Cases

为观察补中益气汤加减口服联合生物反馈治疗气虚下陷型直肠黏膜内脱垂的临床疗效,将50例诊断为气虚下陷型直肠黏膜内脱垂患者,随机分为观察组和对照组各25例。对照组给予生物反馈治疗,观察组在对照组治疗基础上,给予补中益气汤加减口服。对比两组肛门坠胀评分(VAS)、肛门直肠压力、焦虑(SAS)及抑郁(SDS)评分、临床疗效及安全性。结果显示,两组治疗后直肠黏膜内脱垂相关症状均有改善,但观察组VAS评分为(2.12±1.54)分,低于对照组的(3.52±1.85)分(P<0.05);观察组SAS评分为(39.28±5.20)分,低于对照组的(49.44±4.90)分(P<0.05);观察组SDS评分为(40.28±5.20)分,低于对照组的(49.92±4.31)分(P<0.05);观察组治疗后直肠静息压、肛管静息压及最大缩榨压均小于对照组(P<0.05);观察组总有效率为92.00%,高于对照组的60.00%(P<0.05);两组均未出现不良反应。结果表明,补中益气汤加减口服联合生物反馈治疗气虚下陷型直肠黏膜内脱垂疗效显著,可明显缓解肛门坠胀不适及焦虑抑郁症状,降低肛门直肠压力,值得临床推广。

This study was to observe the effect of Buzhong Yiqi Tang(a TCM formula,shortly named as Tang)combined with biofeedback in the treatment of internal rectal prolapse(IRP)with qi deficiency and sinking,randomly divided 50 IRP patients with qi deficiency and sinking into control group(25 cases,received biofeedback therapy)and observation group(25 cases,biofeedback therapy plus orally administering Tang);then,compared both groups’patients’score ratings on anal bearing-down&swelling(VAS),SAS ratings,SDS ratings;and ano-rectal pressure,as well as clinical effect and safety.As results,both groups’relevant symptoms of IRP got all improved,but in VAS ratings,in SAS ratings,in SDS ratings,in ano-rectal pressure(including rectal rasting pressure,anal resting pressure,and max.squeeze pressure),and total effective rate observation group was significantly and respectively lower[(2.21±1.54)vs(3.52±1.85)points],lower[(39.28±5.20)vs(49.44±4.90)points],lower[(40.28±5.20)vs(49.92±4.31)points],smaller,and higher(92.00%vs 60.00%)than control group(all,P<0.05);in both groups no unward reactions were found.Results show that Tang plus biofeedback therapy for IRP with qi deficiency and sinking has significant effect,can greatly relieve anal bearing-down&swelling feeling,as well as anxiety and depression symptoms,reduce ano-rectal pressures,thus is worthy to be popularized clinically.
