集束化护理对机械通气患者呼吸机相关性肺炎的预防作用 被引量:19

The application effects of cluster nursing in prevention of ventilator-associated pneumonia and its effects on the incidence of VAP

摘要 目的分析呼吸机相关性肺炎(VAP)预防中集束化护理的应用效果。方法以2019年1月至2020年1月收治的机械通气患者为对象260例,根据随机数字表法将其分为2组。对照组130例予以常规护理,试验组130例予以集束化护理,对比分析2组护理后VAP发生情况、依从性、干预效果及血气分析指标。结果试验组护理后VAP发生率为3.85%,比对照组的12.31%更低,而试验组护理依从性明显高于对照组,差异有统计学意义(P<0.05);护理后,试验组ICU入住时间以及机械通气时间均比对照组短,差异有统计学意义(P<0.05);试验组血氧饱和度及动脉血氧分压水平均比对照组高,而二氧化碳分压水平比对照组低,差异有统计学意义(P<0.05)。结论集束化护理应用于机械通气患者可有效预防VAP,提升患者依从性,缩短其ICU入住时间及机械通气时间,并改善患者血气分析指标。 Objective To investigate the application effects of cluster nursing in prevention of ventilator-associated pneumonia and its effects on the incidence of VAP.Methods A total of 260 patients who were treated by mechanical ventilation in our hospital from January 2019 to January 2020 were enrolled in thestudy,who were randomly divided into two groups according to the random number table method,with 130 cases in each group.The patients in control group were treated by routine nursing,however,thepatients in experimental group were treated by cluster nursing.After nursing intervention,the incidence of VAP,treatment compliance,intervention effects and blood gas analysis indexes were observed and compared between the two groups.Results After nursing intervention,the incidence rate of VAP in experimental group was 3.85%,which was significantly lower than that(12.31%)in control group,however,the treatment compliance was superior to that in control group(P<0.05).After nursing,the ICU stay time and mechanical ventilation time in experimental group were significantly shorter than those in control group(P<0.05).Moreover the the levels of blood oxygen saturation and arterial blood oxygen partial pressure in experimental group were significantly higher than those in control group(P<0.05),but the carbon dioxide partial pressure levels were significantly lower than those in control group(P<0.05).Conclusion The application of cluster nursing for patients undergoing mechanical ventilation can effectively prevent the incidence of VAP,improve patient’s compliance,shorten ICU stay time and mechanical ventilation time,and improve blood gas analysis indexes.
